Right now we are looking for a Registered Nurse to work with our Kidney Care Clinic Team.

 

Hiring Salary Range: $34.24-$49.02

Location: Humber River Hospital – Church Site, Cross site coverage may be required
Hours of work: Available for day and some evening shifts Monday to Fridays (subject to change)

Responsibilities

  • Highly motivated individual capable of working independently as well as with the inter-professional team to coordinate a plan of care for patients having a Chronic Kidney Disease (CKD)
  • Functions as the primary contact and resource for new renal patients in the Nephrology Program
  • Works in collaboration with the Nephrology team at the Kidney Care Clinic to ensure that all patients have the most appropriate body access as well as to promote independent dialysis within this patient population
  • Advocates for patient choice and shared decision making by providing patients and families with information on treatment modalities and assisting patients and families in achieving an optimal level of self-care
  • Advocates for the individual needs of patients by incorporating critical analysis involving observation, evaluation and discussion with patient and families
  • Demonstrates the ability to customize teaching approaches based on individual learning needs
  • Facilitates communication between patients/families and members of the health care team
  • Attends and participates in rounds as required
  • Modality Home Hemodialysis and Peritoneal dialysis teaching
  • Participates in hospital committees, administrative and Nephrology clinical planning activities

Requirements

  • Current registration with the College of Nurses of Ontario,
  • BCLS certification
  • Bachelor’s degree in Nursing
  • CNeph Certification preferred. Required to complete CNeph certification or equivalent within 18 months from the hiring date.
  • Kidney Care Clinic and Nephrology dialysis experience required
  • Knowledge and experience with the development, implementation and evaluation of educational programs, curriculum and client education for CKD, PD, hemodialysis and post kidney transplant patients and their families required
  • Demonstrated knowledge of adult teaching and learning principles and the ability to effectively provide health teaching to individuals and groups required
  • Demonstrates excellent patient assessment skills
  • Demonstrated ability to collaborate/liaise with inter-professional team members, inter-agency groups and appropriate community resources required
  • Demonstrates excellent organizational skills
  • Demonstrates excellent interpersonal communication skills with the ability to work with a multidisciplinary team including other health team members, patient/family
  • Demonstrated ability to problem solve and handle crisis situations, client/family issues and interdepartmental conflicts in a calm and capable manner required
  • Commitment to team work and role modeling positive professional interpersonal relationships required
  • Ability to work with minimal supervision required
  • Must be available to perform home assessments and home visits as part of the renal replacement modality plan
  • Must be available to provide education to admitted ESRD patients to advance their renal replacement plan
  • Excellent organizational, leadership and clinic leadership skills required with excellent verbal and written communication skills
  • Maintains competency in Nephrology and in Leadership through ongoing education and practice
  • Demonstrated computer skills using Microsoft office (Word, Excel, Access)
  • Ability to be flexible and effectively work in busy environment with changing priorities and deadlines
  • Knowledge of second language an asset
  • Excellent attendance record
