We currently have an exciting opportunity for an experienced and committed Clinical Manager, Home Programs, Kidney Care Clinic, Transitional Care Unit, 21 station incentre dialysis program and Allied Staff to join our Nephrology Program at the Church Site. 

 

In consultation with the Program Director, Clinical Practice Leader and Team members, the Manager will contribute significantly to the successful development and implementation of the clinical services, functional and strategic plan for the specialized unit. As an integral team member of the Nephrology Program, there is an opportunity to lead the Program by utilizing a chronic disease prevention and management (CDPM) and self-management approach and philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network, and strives to achieve the targets of the Ontario Renal Plan III:

  • Empower and support patients to be active in their care
  • Integrate patient care throughout the kidney care journey
  • Improve patient access to kidney care

 

RESPONSIBILITIES:

 

Reporting to the Program Director, the Clinical Manager will:

  • Provide effective, efficient and safe patient services through human, financial, and environmental resource management.
  • Support a model of patient centered care that is based on best practice, program and corporate standards, and continuous quality improvement.
  • Build a professional practice climate and facilitates an interdisciplinary approach to patient care.
  • Collaborate with internal and external stakeholders and community partners to plan, implement and monitor delivery of patient and family centered programs and services.
  • Provide leadership in advancing the goals and objectives of the Nephrology Program and Ontario Renal Network as well as supports the program and organization’s strategic priorities.
  • Coach and mentor staff to navigate successfully through change.
  • Manage all aspects of employee relations including recruitment, retention, attendance and performance management in collaboration with Human Resources and Labour Relations.

 

QUALIFICATIONS AND EXPERIENCE:

  • Current registration with the College pertinent to the Regulated Health Profession and a Baccalaureate Degree in Nursing required. Master’s in Nursing, Health Science or related field required. 
  • Master’s in Nursing, Health Science or related field required. Candidates who are actively pursuing their Masters will be considered.
  • Minimum of five years relevant clinical experience, in nephrology preferred, and a minimum of three years leadership experience in an acute care setting.
  • Demonstrated and effective critical thinking, decision making and problem solving skills within a fast paced, complex and continually changing environment.
  • Demonstrated success in leading change, building high functioning teams and effective working relationships.
  • Skills and experience in strategic planning, project management, and effective resource management and utilization.
  • Excellent attendance and discipline free record required.
