We currently have an exciting opportunity for a Nurse Practitioner with our Bariatric, Surgical Program.

Reporting Relationship: Manager, Bariatrics

Hourly Rate Range: $61.51-$76.82

Schedule Requirements: Days, evenings and weekends (subject to change as per Article 3 – Management Rights. 

Location: Wilson Site (subject to change as per Article 3 – Management Rights.)

Employee Group: ONA 

 

Responsibilities:

  • Autonomously diagnose, order and interpret diagnostic tests and prescribe pharmaceuticals within scope of practice as it relates to the Bariatric Program; includes micronutrient supplementation
  • Update policies, procedures and Standards of Care for the Bariatric Program
  • Ability to support patients by providing clinical expertise, guidance and education
  • Assist in the coordination of activity between the surgical and medical clinics
  • Work collaboratively with the interdisciplinary team and with the physicians
  • Communicate abnormal findings with the team
  • Conducts health assessment and focused physical exam on high-risk patients; identifies normal and abnormal findings through investigation and liaises with PCP as needed and provides appropriate referrals
  • Facilitate the transition of patients post-operatively over to their Primary Care Providers
  • Facilitate team rounds and assist in the provision of comprehensive medical care for the bariatric patient
  • Identifies researchable questions and participates in research initiatives
  • Providing education on matters related to bariatric surgery, weight loss and health promotion
  • Formulates differential diagnoses, orders and/or performs screening and diagnostic investigation based on presenting health concern and treating using evidence based best practices and triaging post-operative for complications warranting ED visit and liaising with surgeons and internist as needed.  This includes observing for post-bariatric surgery complications and treating appropriately
  • Prevents unnecessary ED visits
  • Ability to incorporate knowledge of diversity, cultural safety and the determinants of health in the assessment, diagnosis and therapeutic management of patients and the evaluation of outcomes
  • Identifies educational programs for the care team.
  • Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within multidisciplinary teams
  • Ability to work days, evenings, and weekends as required

 

Qualifications:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the extended Class (Primary Health Care or Adult) and entitled to practice with no restrictions. 
  • Member of the Registered Nurses Association of Ontario preferred         
  • Minimum 2 years experience as an NP in primary care, long-term care, acute care or continuing care.
  • CDE and CBE certification preferred
  • Knowledge of Bariatric, community resources, crisis work and a working knowledge of the Regulated Health Profession Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act. 
  • Demonstrated knowledge of the principles of adult education
  • Experienced in Program development and evaluation and analysis of statistics
  • Demonstrated experience and proficiency in the provision of effective clinical consultation and assessment.
  • Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agencies
  • Excellent verbal and written presentation skills
  • Excellent problem solving ability and critical thinking skills and able to work independently and in collaboration with interdisciplinary team
  • Excellent computer skills using Office 365
  • Knowledge of Quality Improvement process, data analysis and program development
  • Demonstrated commitment to excellence in patient/family centred care
  • Able to work independently with minimal support.
