We currently have an exciting opportunity for a Registered Nurse (CDE) to join our Diabetic Education Centre for the position of Diabetes Educator.

Responsibilities:

  • Conduct thorough nursing assessments of clients with diabetes, taking into consideration ethno-cultural backgrounds and the social determinants of health
  • Collaborate with the Registered Dietitian and the Endocrinologist in development of client-focused health maintenance plans related to diabetes while utilizing a holistic and inter-disciplinary approach
  • Use a variety of teaching methods and principles of adult education to engage with clients to enhance diabetes self-management in group and individual settings
  • Ensure continuity of care by maintaining up to-date medical records, communicating with the client’s primary care provider and other service providers, and making referrals where appropriate
  • Advocate on behalf of clients with respect to systemic issues as they arise
  • Regular monitoring of clients within scope of practice
  • Act as an inpatient resource for patients that have a new diagnosis of diabetes or are experiencing challenges with diabetes management
  • Engage in health promotion, outreach activities and diabetes education services in the community
  • Participate in program planning, evaluation and resource development based on a client-centered, inter-disciplinary approach
  • Act as a diabetes resource to staff members and community stakeholders
  • Contribute to policy and protocol development in relation to clinical practice delivery and/or professional practice
  • Participate in chart reviews and case conferences
  • Maintain and develop professional competencies

 

Requirements:

  • Current registration with the College of Nurses of Ontario in good standing as a Registered Nurse
  • Minimum 2 years working experience in a Diabetes Education Program.
  • Certification as a Diabetes Educator is required
  • Experience in program development, implementation, monitoring and evaluation
  • Experience with individual and group facilitation of education utilizing various adult learning principles
  • Understanding of and demonstrated ability to work effectively in a collaborative, inter-disciplinary team environment
  • Knowledge of chronic disease management beyond diabetes management required.
  • Excellent organizational, communication and interpersonal skills and demonstrated ability to take initiative
  • Sensitivity to and awareness of cultural, racial, economic and socially diverse communities
  • Experienced in the use of computers, various software applications and EMR systems
  • Proficiency in a second language used in the community served would be an asset
  • Excellent attendance and discipline free record required.
