Overview

 

The role of the Social Worker in the Humber River Family Health Team requires a thorough knowledge of the social determinants of health and mental health issues in a primary care context and the ability to function effectively in a team environment with physicians, other interdisciplinary health professionals and administrative staff.

 

This role also requires a high level of interpersonal skills to deal with patients and their families in a holistic and compassionate manner.

 

Employment Status- Full Time
Duration: permanent

Hours of Work: Must be available for 8 hour shifts, days, evenings (subject to change)

Location: Humber River Health, Wilson Site

Area of Focus:  Schulich Family Medicine Teaching Unit & Integrated Care Services

Reporting Relationship: Manager, Schulich Family Medicine Teaching Unit & Integrated Care Services

 

 

Responsibilities:

 

  • Conducts comprehensive assessments for outpatients in ambulatory settings, addressing referral source concerns, identifying gaps in access to resources for self-management of physical and mental health, and evaluating patients and family members to determine psychosocial needs and barriers to treatment and care.
  • Collaborates with clients, caregivers, and referral source to develop appropriate treatment plans
  • Maintains awareness of community resources, connects patients to appropriate services, and recommends solutions for identified gaps.
  • Provides feedback on outcomes and helps patients and providers navigate healthcare and community systems.
  • Provides case management and expertise in crisis services and mental health
  • Works in a manner to ensure safety to oneself, clients, colleagues and the work environment
  • Acts as a patient and family advocate, ensuring their rights, needs, and values are recognized
  • Acts as a resource to clinic staff and the inter-professional team in assessing, planning and managing the psychosocial care of patients
  • Participates in the interdisciplinary approach to care
  • Flexibility in caseload and program assignment:Provide crisis intervention in collaboration with team
  • Assess and address the impact of social determinants of health on the wellbeing of patients and their families, by utilizing an anti-oppressive framework
  • Counsels patients and families to help them clarify and meet their goals and provides services to aid in their attainment
  • Informs patients and families of appropriate community resources and assists in their coordination
  • Provides services for walk in appointments, where possible.
  • Manages all patients referred by the physicians and nurse practitioners,and effectively triage and assign referral priority ratings
  • Effectively support patients to manage their clinical plans including healthy choices, healthy lifestyles, appointments, medication as necessary, and self-management of care.
  • Collaborates with the providers to develop and deliver programs, providing individual and group sessions on emotional regulation, mindfulness, and distress tolerance.
  • Perform other related duties as assigned by the Manager.

 

Qualifications:

  • MSW obtained from an accredited school of Social Work required
  • Current membership with OASW and OCCSWSSW, or eligibility to become a member required.
  • 2 to 3 years of experience within primary care or family health team is an asset
  • CBT, mindfulness, and trauma informed experience preferred
  • Experience with care across the lifespan an asset
  • Computer skills required, specifically, ability to manage electronic recording, workload and meet submission timelines.
  • Demonstrated knowledge of the acute, sub-acute and community sector.
  • Knowledge of community resources an asset.
  • Demonstrated organizational skills to set clinical and non-clinical priorities in a fast paced environment.
  • Demonstrated ability to work independently and in a team environment.
  • Demonstrated excellence in interpersonal skills.
  • Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, families, peers, other healthcare team members and volunteers.
  • Excellent decision making and advocacy skills.
  • Demonstrates cross cultural awareness and sensitivity.
  • Proficiency in a second language used in the community served would be an asset
  • Excellent attendance and a discipline free record are a requirement
