The Resource Nurse, Healthy Living Clinic has primary day-to-day responsibility for patient care; the safety and quality of the experience for both patients and staff; and flow outcomes of the patient care area.

 

The Resource Nurse provides clinical leadership in the planning, implementation, and evaluation of care for patients in the program in a manner consistent with the College of Nurses of Ontario Standards of Professional Practice, the Regulated Health Profession Act, and the Mission, Vision and Values of Humber River Health.

 

Within this role, the Resource Nurse is accountable for contributing to the delivery of the Humber River Health's Strategy and must demonstrate an awareness of and responsibility for actively promoting and supporting patient- and family-centred engagement and care.

Responsibilities:

 

Clinical Practice

 

  • Supports the timely and accurate triaging of referrals
  • Ensures accurate and complete communication regarding patient related issues from shift to shift assists manager/educator in identification of any areas of clinical practice that need development ensures a safe environment for patients, visitors, staff
  • Ensures appropriate follow up of any issues which may have arisen from previous shift i.e. patient/family concerns, incidents, change in patient acuity, etc.
  • Awareness of the plans of care for all patients including patient status, special procedures/tests and referrals.
  • Ensures patient assignments, when applicable, are based on patient needs, scope of practice / skills and knowledge of staff
  • Supports clinic flow by taking on a patient care activities and tasks

Patient Care

 

  • Assesses, plans, implements and evaluates patient care in alignment with the RN scope of practice outlined by the College of Nurses of Ontario.
  • Integrates best practice guidelines and evidence based research into clinical practice related to the geriatric population
  • Facilitates movement through transitions of care, ensuring patients are connected with the appropriate services, resources and supports within HRH and in the community
  • Provides health education for patients and families regarding chronic disease management, diagnostic tests and prescribed treatments
  • Liaises with the patient’s primary care physician or primary care provider and community agencies as required throughout the care process
  • Identifies and prioritizes not only the physical needs, but also the psychological, social and spiritual needs of the patient
  • Ensures that the patient or caregiver is included in development of the care plan and understands the medical advice or recommendations.
  • Works collaboratively with the interdisciplinary team to support the ongoing functioning of the outpatient Geriatric Clinic related to referrals, consultations and follow-ups
  • Maintains the health record according to CNO documentation standards and Humber River Health Policies and Procedures
  • Adjusts staffing, assignments throughout the day as required, and anticipates adjustments for future days
  • Acts as a clinical resource to staff as required
  • Facilitates effective communication among interdisciplinary team members and reports problems affecting patient care/the interdisciplinary team/the clinic to the manager
  • Ensures patient care plans are reflective of current patient status facilitates interdisciplinary team planning for patients
  • Encourages staff to participate in liaising with physicians and sharing information with other interdisciplinary team members
  • Ensures patient care planning is conducted in an efficient and effective manner using hospital and community resources as necessary
  • Facilitates patient tests/procedures within the hospital, or to another facility

 

Educational Responsibilities

 

  • Keeps abreast of new developments in Geriatric Nursing and related evidence-based advancements with the intention of integrating into clinical practice
  • Engages in continuous learning activities such as: attendance at in-services, seminars and courses, sharing information obtained at educational sessions with colleagues.
  • Assists Manager/Clinical Practice Leader in identifying staff educational needs in conjunction with Manager/ Clinical Practice Leader, co-ordinates staff attendance at in-services shares nursing knowledge and expertise with staff through incidental teaching
  • Collaborates with nursing staff and interdisciplinary team members to ensure patient teaching programs are implemented Identifies need for patient education resources/materials
  • Participates in nursing education program by collaborating with Manager/ Clinical Practice Leader, and instructors from nursing faculties to help identify preceptors and appropriate patient assignments for students/orientees
  • Facilitates orientation of orientees/students
  • Assigns staff to attend mandatory in-services

 

Non-Clinical Responsibilities

 

  • Informs manager of issues related to patients, families, clinic or staff requiring escalation
  • Leads daily Patient Quality & Safety Huddles with the interdisciplinary team Leads and provides resource for staff to provide factual, timely information/documentation regarding patient/staff incidents/equipment malfunction
  • Demonstrates the ability to navigate and perform computer competencies
  • Ensures equipment in the clinic is maintained in good working order
  • Participates in Continuous Quality Improvement through root cause analysis and solution implementation
  • Ensures safety issues are dealt with promptly by initiating appropriate stakeholder response and requisitions
  • Assists Manager with staffing (i.e. replacement for sick calls, schedule changes, etc.) ensuring appropriate composition of staff is available to meet patient care needs for the upcoming weeks
  • Participates in program planning
  • Other duties as assigned by the Manager

 

Requirements

 

  • Experience with the assessment of various geriatric syndromes
  • Demonstrates knowledge of geriatric nursing management, cognitive tests, delirium, dementia and depression (3Ds)
  • Ability to adapt and effectively work in a fast-paced environment with changing priorities and time sensitive targets
  • Must possess excellent organizational skills
  • Exhibit advanced assessment skills and an ability to identify and react quickly to a deteriorating patient. Health Assessment course would be an asset.
  • Demonstrates excellent patient and family teaching skills that encompass adult learning principles
  • Demonstrates effective problem solving skills and an ability to work well independently with minimal supervision
  • Highly effective interpersonal and communication skills are required
  • Sensitivity to and awareness of cultural, racial, economic and socially diverse communities
  • Excellent attendance and discipline free record required.

 

Qualifications

  • Baccalaureate degree in Nursing preferred
  • Registered with the College of Nurses of Ontario in good standing
  • Maintains current speciality certifications for related clinical area
  • BCLS certification or equivalent
  • Two years recent/related experience in clinical specialty or equivalent education and experience within the last five years
  • Outpatient Clinic experience is an asset
  • Related CNA Certification, specialty certification (if appropriate) from accredited program preferred
  • Leadership experience
