Humber River Health is seeking a full time Program Director, Post Acute and Reactivation Care Centres to be part of our dynamic team and lead the way as we continue our journey towards high reliability care!

 

 Reporting to the Vice President, Post Acute and Health System Partnerships, the Program Director, Post Acute and Reactivation Care Centres will provide strategic leadership oversight for the delivery of care aligned with corporate goals and objectives.

The Program Director’s portfolio will include current and future HRH inpatient services at the Reactivation Care Centres , including planning and operationalizing the innovative Acute Behavioural Assessment and Management unit. In addition, the portfolio will include  Transitions, Social Work Team, Schulich Family Medicine Teaching Unit and Integrated Care Services.

 

In your role, you will be responsible for partner hospital relations management and will engage and collaborate with post-acute partners and other key stakeholders on development of services and pathways with the focus on integrated care and emergency room visit avoidance.

 

In collaboration with Program leaders, you will develop and promote the strategic direction of the Program’s vision, goals and objectives using evidence based best practices that align with the hospital’s strategic plan meeting legal, regulatory, and other priorities and requirements. You will evaluate your strategic plans by implementing performance indicators and the quality improvement processes you lead will ensure quality monitoring, avoid and mitigate risks while improving quality and access to care for patients and the hospital. You will foster cultural awareness and sensitivity among patients, staff, and the external community.

Reporting Relationship:  Vice President, Post Acute and Health System Partnerships,

 

Key Responsibilities:

  • Collaborate with Chief/ Physician Director  to develop and promote the strategic direction of Post Acute services (i.e. vision, goals, and objectives) using evidence based best practices aligned with the hospital’s vision and strategic direction to meet legal, regulatory, and other priorities and requirements (e.g. OH and MOH targets); evaluate impact of strategies on employees, the organization, and patients
  • Develop annual operating plans aligned with hospital program/service plans and goals; consider opportunities and risks associated with proposed options; select courses of action with the highest probability of success
  • Integrate clinical program functions, philosophy, process, policies with other program areas to improve patient care quality outcomes and patient/family experience (e.g. inter-program patient flow)
  • Plan service delivery growth; lead initiatives to acquire new or increased funding resources
  • Develop business ventures based on market and business drivers; build on existing market strengths while balancing business risk (i.e. take calculated risks) to pursue new ventures; pilot and evaluate new patient care initiatives and processes
  • Set the program’s strategic direction in collaboration with Chief/ Physician Director to support and promote the use of best practices
  • Monitor and governs compliance with professional, accreditation, ethical, and legal standards
  • Manage escalated issues independently and in collaboration with clinical team; take action to resolve issues and conflicts, and maintain positive relationships
  • Develop relationships with community partners and regulatory bodies to exchange information, best practices, and professional standards that contribute to HRH’s image and growth

 

Career Experience and Core Competencies:

  • Regulated Health Care Professional in good standing with respective health professional regulatory body in Ontario and/or expertise working in a complex clinical setting
  •  University degree in a health-related field
  • Master's in Health or Business Administration required
  • A minimum of (7) seven years of clinical experience in an acute care hospital required
  • A minimum of (5) five  years management experience required
  • Canadian Health Executive (CHE), and Leadership courses; LEAN certification, preferred
  • Proficiency in MS Office (e.g. Outlook, Word, Excel, PowerPoint, Access, etc.)
  • Knowledge and applied experience in unions, budget management and business acumen
  • Experience in negotiation, interpersonal, communication, problem solving, analytical, issue management, diplomacy, political acuity, collaboration
  • Experience with Meditech, Hospital automation and digital technology solutions preferred
  • Experience in Lean or population-based planning, vendor management, project/Change Management, and committee management preferred
