Position Responsibilities:
- Practices and has proficiency in all areas of renal disease management including in-patient nephrology, pre-dialysis clinic, home dialysis, and in-center hemodialysis.
- Provides psychosocial assessment, crisis intervention, discharge planning and counseling to nephrology patients and their families.
- Acts as a resource to physicians and interprofessional team members in assessing, planning and coordinating the psychosocial care of patients.
- Collaborates with patients and family members to determine psychosocial needs, strengths, limitations and barriers to treatment, and comprehensive discharge needs.
- Develops an understanding and creates partnerships with community resources to support effective discharge planning in collaboration with the interprofessional team members.
- Establishes a plan of care with patient and family members to encourage their active participation in goals setting and develops a plan of care for service delivery.
- Plans, organizes, co-ordinates community resources which have been identified to best support and address the patient’s and family’s needs, as deemed applicable.
- Counsels patients and families to help them adjust and cope with living with CKD.
- Acts as patient and family advocate ensuring their needs, options, and values are recognized within the social work scope of practice.
- Effectively communicates with patients, families, and the interprofessional team to prevent and mitigate conflict.

Qualifications:
- MSW obtained from an accredited school of Social Work required.
- Current membership with OASW and OCCSWSSW, or eligibility to become a member required

- Experience in the acute and/or sub-acute sector system, patient flow and discharge planning is an asset and preferred.

Responsibilities:
- As a member of the health care team, this position assumes the responsibility for assessment, planning, implementation, care coordination and evaluation of patients who referred for diabetes care and management.
- Contributes to the development of evidence based professional practice model of care.
- Flexibility in caseload and program assignment.
- Able to manage patient caseload in a busy acute care setting.

Requirements:
- Bachelor’s degree in Food and Nutrition
- M.Sc. in Nutrition or related field preferred

Position Responsibilities:
The Student Nurse Extern role is an unregulated healthcare worker providing direct personal support to HRH patients. Student Nurse Externs will be responsible for the provision of basic care (activities of daily living such as bathing, toileting, feeding, etc.) needs for up to 6 patients each. The Student Nurse Extern may work under the supervision of both Registered Nurses and Registered Practical Nurses in accordance to department policies and procedures. The Student Nurse Extern will also be responsible for the documentation of activities of daily living (ADLs), such as intake and output, and relevant communication for patients in which they have provided care for. The Student Nurse Extern will not perform or be delegated any regulated acts (medication administration, vital signs, assessments, etc.). The Student Nurse Extern will immediately report any clinical concerns to their direct supervisor.
Qualifications:
- The successful candidate must be currently enrolled years 2-4 of their nursing program of studies

Right now we’re looking for a Data Integrity Analyst to work with our Surigcal Program, National Surgical Quality Improvement Program (NSQIP)
Reporting Relationship: Manager, Bariatrics
Hours of Work: Days, Monday to Friday
Responsibilities:
- Prepare and submit monthly Activity Level Reports (ALR) in collaboration with IS&T/Health Records
- Develop quality flow and reporting processes (e.g. QA process map) in consultation with Manager
- Analyze and coordinate changes to meet newly updated reporting requirements\
- Update data elements file and data source maps in database
- Adjust data with Manager approval to respond to issues
- Discuss changes to funder strategic plans with Manager to analyze impact and transfer knowledge of changes
- Creating meaningful utilization reports using various systems (WTIS, iport, Meditech, abstracted and financial data) to package key monitoring information that would be user friendly and available in real time.
- Providing regular statistical and narrative data on Wait Time activities with interpretation to support the various clinical programs.
- Providing regular statistical and narrative data on Decision Support activities with interpretation to support the various clinical programs.
- Educating health care providers in the benefits of appropriate documentation and identifying the impact of documentation gaps as it relates to wait time and financial Information.
- Assisting in the procurement, testing and implementation of valid and reliable tools for clinical resource utilization monitoring and analysis
- Working in collaboration with the Information Systems Specialist to maintain/update/change dictionary entries of Wait Time Information System as required.
Requirements:
- Completion of diploma or degree in health sciences, health administration, epidemiology, information management, or related programs. A combination of experience and education will be considered.
- Minimum three years experience in database systems and activity level reporting.
- Sound knowledge of medical terminology and documentation systems an asset.
- Demonstrated report writing capabilities.
- Demonstrated computer knowledge/expertise in MS Office i.e. Power point, Excel, Access, Word - spreadsheets, graphs and databases with report writing skills.
- Experience in CIHI data and ICD-10 coding preferred.

Responsibilities
- Highly motivated individual capable of working independently as well as with the inter-professional team to coordinate a plan of care for patients having a Chronic Kidney Disease (CKD)
- Functions as the primary contact and resource for new renal patients in the Nephrology Program
- Works in collaboration with the Nephrology team at the Kidney Care Clinic to ensure that all patients have the most appropriate body access as well as to promote independent dialysis within this patient population
- Advocates for patient choice and shared decision making by providing patients and families with information on treatment modalities and assisting patients and families in achieving an optimal level of self-care
- Advocates for the individual needs of patients by incorporating critical analysis involving observation, evaluation and discussion with patient and families
- Demonstrates the ability to customize teaching approaches based on individual learning needs
- Facilitates communication between patients/families and members of the health care team
- Attends and participates in rounds as required
- Modality Home Hemodialysis and Peritoneal dialysis teaching
- Participates in hospital committees, administrative and Nephrology clinical planning activities
Requirements
- Current registration with the College of Nurses of Ontario,
- BCLS certification
- Bachelor’s degree in Nursing
- CNeph Certification preferred. Required to complete CNeph certification or equivalent within 18 months from the hiring date.
- Kidney Care Clinic and Nephrology dialysis experience required
- Knowledge and experience with the development, implementation and evaluation of educational programs, curriculum and client education for CKD, PD, hemodialysis and post kidney transplant patients and their families required
- Demonstrated knowledge of adult teaching and learning principles and the ability to effectively provide health teaching to individuals and groups required
- Demonstrates excellent patient assessment skills
- Demonstrated ability to collaborate/liaise with inter-professional team members, inter-agency groups and appropriate community resources required
- Demonstrates excellent organizational skills
- Demonstrates excellent interpersonal communication skills with the ability to work with a multidisciplinary team including other health team members, patient/family
- Demonstrated ability to problem solve and handle crisis situations, client/family issues and interdepartmental conflicts in a calm and capable manner required
- Commitment to team work and role modeling positive professional interpersonal relationships required
- Ability to work with minimal supervision required
- Must be available to perform home assessments and home visits as part of the renal replacement modality plan
- Must be available to provide education to admitted ESRD patients to advance their renal replacement plan

- Maintains competency in Nephrology and in Leadership through ongoing education and practice

Position Responsibilities:
- All aspects of care for patients receiving hemodialysis with a primary nursing focus
- Providing education and follow-up to all hemodialysis patients
- Providing primary care to assigned patient that includes but not limited to ensuring plan of care is developed in collaboration with multidisciplinary team, updated and communicated with team in a timely manner
Qualifications:
- Current RN registration with the College of Nurses of Ontario
- BCLS certification upon hire to show proof of valid current certification (Registered Nurses currently employed by HRH are exempted).
- BScN preferred
- Minimum two year acute Medical / Surgical experience, required.
- Minimum two years recent Hemodialysis experience is required.
- Nephrology courses 1 to 4 completed or commitment to complete within an agreed period of time.
- Certification in Nephrology an asset or commitment to complete within an agreed period of time
- Patient assessment course and demonstrated good patient assessment skills
- Experience in peritoneal dialysis an asset
- Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients, family members and visitors
- Demonstrated good patient/family teaching skills
- Ability to prioritize and complete assignments in a reasonable length of time:
- Demonstrated sound judgement, good observation and patient assessment skills, tact and empathy;
- Demonstrated ability to perform appropriately in emergency situations;
- Proven ability to perform a leadership/preceptorship role;
- Physically capable of performing the essential duties of the position;
- Demonstrated understanding of and commitment to Humber's comprehensive safety programs and practices, including staff patient, and environmental safety, and participation in regular in-service education in this area; and

Responsibilities:
- Acting as the first point of contact for all computer and related hardware, application and telecom related requests for IT service and support.
- Resolve technical issues related to servers, computers, printers, computer peripheral devices and telecommunication devices.
- Troubleshoot and resolve issues with Microsoft Windows operating system, Microsoft Office and health information applications.
- Recording, categorizing and logging requests for service using a central ticket management solution;
- Leveraging available tools (SMS, remote support utilities, knowledge base and other sources of documentation) to resolve incidents in an efficient and effective manner
- Transferring and escalating incidents and problems to 2nd/3rd level support resources and following up with ticket owners to ensure resolution in a timely manner;

- Upgrading existing PC Hardware and software.
- Participates in the rotating On-call support program.

Requirements:
- Successful completion of University and/or Community College diploma in a health-related or computer technology program.
- Minimum 3 years’ experience in a Technical Service Desk role is required.
- Minimum 3 years’ experience with expert level proficiency in Microsoft, Windows 7, Windows 8 and Microsoft Office 2013, MS Server 2008/12, MS SQL 2008/12, and Citrix Desktop is required.
- Experience with Meditech and other healthcare information applications is an asset.
- Experience with troubleshooting and resolving computer hardware, printer hardware, peripherals, and E-Mail Systems issues is required.
- Experience in troubleshooting Local and Wide area networks and is an asset.
- ITIL Foundation experiences and/or certification is an asset.

Our Critical Care Unit offers state of the art care for cardiac, medical and surgical critically ill patients.
Our Critical Care Registered Nurses are charged with the care of the most vulnerable patients and their families. Using the latest technologies and working with interdisciplinary teams they deliver compassionate paitent-centred care.
Reporting Relationship: Manager of Critical Care
Hourly Rate Range: $34.24- $49.02
Schedule Requirements: Must be available days, nights, weekends and statutory holidays.
Location: Wilson
Position Responsibilities:
- Care of acutely ill cardiac medical and surgical patients.
- Care of patients requiring Hemodynamic and ventilation monitoring by means of invasive and non- invasive approaches.
- Care planning and coordination of care with various team members embracing patient centered care.
- Utilization of computerized systems for documentation of patient care.
- Participation in project teams or Hospital committees as required.
Qualifications:
- Current RN registration with the College of Nurses of Ontario required.
- Current BCLS certification required; ACLS preferred.
- Minimum 2 years Critical Care experience.
- Critical Care Certificate required.
- Experience in working on a Nurse Lead Rapid Response Team preferred
- Experience in critical care with hemodynamic monitoring and invasive and non invasive ventilation required.
- Basic computer skills required.
- Good problem solving skills required.
- Demonstrated excellent interpersonal and communication skills with an ability to work with a multidisciplinary team, patients, family members and visitors.
- Excellent organizational skills with an ability to set priorities and meet deadlines.
- Demonstrated good patient assessment skills. Patient assessment course preferred.
