As part of our dynamic team, you can lead the way, as we continue our journey towards high reliability care!   We currently have an exciting opportunity for a Registered Nurse to join our Outpatient Mental Health and Addiction Services   Location:  Wilson Site – Cross Site Coverage may be required – Must be willing to work on all Mental Health Units Hours of work: Day shift Monday to Friday, some evenings may be required Hourly Rate Range: $34.24 - $49.02 Employee Group: ONA    Responsibilities: - Assessment and treatment of patients in the Outpatient Mental Health Service, with special emphasis on those individuals with serious and persistent mental illness. - Participate in clinic activities, including the depot medication injection clinic and clozapine clinic if required. - Collaborate with the Physician Leader, Manager and other stakeholders in the development of new programs for patients and families. - Deliver individual/group therapy and psycho-education for patients and/or their families. - Evaluate and perform psychiatric and physical status history/assessment of patients who are referred to the program and provide follow-up as required. - Facilitate strong working relationship within our internal Mental Health and Addictions programs as well as within our community partners. - Nurses may be reassigned to other mental health units as per the Collective Agreement Requirements: - Current registration with CNO required.  Current BCLS certification required. - BScN or degree in health sciences preferred. - Certificate in Psychiatric Mental Health Nursing required or willingness to obtain when eligible to do so. - Two years’ experience in Mental Health required; with two years’ experience in working with patients who have serious and persistent mental illness preferred. - Excellent assessment skills using a bio-psycho-social model required. - Experience/trained in providing therapy to patients (e.g. interpersonal, cognitive-behavioural, dynamic) is a strong asset. - Excellent familiarity with psychotropic medications and ability to administer depot medication. - An awareness and Knowledge/experience in Trauma Informed Care preferred - Experience with addictions care is an asset, working knowledge of the Mental Health Act. - Familiarity with the interplay between substance use history and mental health symptoms. - Demonstrates strong initiative and possesses excellent organizational skills. - Excellent interpersonal and communication skills and demonstrated ability to work autonomously as well as a part of the interdisciplinary team. - Familiar and comfortable with technology-assisted healthcare delivery, specifically video conferencing using MS teams, OTN, etc - Ability to use mobile technologies such as smartphones and tablets to connect patients to appointments - Excellent attendance and discipline free record required.   Why Humber? This innovative program provides mobile crisis intervention within North West area of the Toronto Police Service Divisions. Graduate Nursing Student Externs will be responsible for the provision and documentation of basic care for patients, and shall work under the supervision of regulated healthcare providers in accordance to department policies and procedures. They shall not perform or be delegated any regulated acts (medication administration, vital signs, assessments, etc.). TIRMIZI Hourly Rate: $25.425 Schedule Requirements: Rotating shifts, 8 or 12 hours, days/evenings/nights/weekends Location: Humber River Hospital - Wilson Site Employee Group: Non-Union   Responsibilities:   Under the supervision of the Primary Nurse or a regulated healthcare professional: - Responsible for the provision of basic care (activities of daily living such as bathing, toileting, feeding, etc.) needs for up to 10 patients each. - Responsible for the documentation of activities of daily living (ADLs), such as intake and output, and relevant communication for patients in which they have provided care for. - Participates in patient assessments - Assists regulated health professionals in routine monitoring (hourly rounding) of assigned patients - Receives report from assigned patients’ Primary Nurse, including any patient specific behaviours or issues to observe and report on. - Identifies and communicates with the inter-professional healthcare team members on patient issues for ongoing patient assignments as necessary, immediately report any clinical concerns to their direct supervisor. - Routinely meets with regulated health professional and / or inter-professional healthcare team as necessary to discuss patient status, safety issues and relevant patient or family information - Collaborates to develop and refine competency in understanding the development and management of patient care planning in preparation for transitioning into temporary registered position. - Participates in all required staff meetings as scheduled - Assists unit Clinical Practice Leader in the collection and identification of practice needs / gaps to develop educational strategies, plans or content, in consultation with the unit leadership team and clinical staff - Assists unit Clinical Practice Leader in the development and evaluation of formal and informal educational programs and materials - Supports and facilitates activities related to Best Practice Spotlight Organization in collaboration with unit CPL, manager and BPSO coordinator.   Qualifications:   - Nursing student in completion of their final year of study in a College of Nurses of Ontario (CNO) approved Baccalaureate Nursing (BScN or BN) program and Practical Nursing (PN) program, prior to obtaining CNO registration - Current CPR Certification - Ability to provide basic assistive care with frequent supervision and ongoing mentoring from a Registered Nurse or Registered Practical Nurse - Ability in assessing difficult situations and using good judgement - Ability to accurately and effectively communicate with the inter-professional healthcare team members - Excellent customer service skills; strong interpersonal and communication skills, ability to use good judgment in assessing difficult situations - Must be a team player (works well with others) - Demonstrates understanding of the use of technology - Mandatory compliance with HRH training- Privacy & Confidentiality requirements, WHMIS, Fire & Safety, hand hygiene modules - Good work attendance Please be advised that in order to be eligible for employment at Humber River Hospital, all new hires must have received the full series of a COVID-19 vaccine or combination of COVID-19 vaccines approved by Health Canada (e.g., two doses of a two-dose vaccine series, or one dose of a single-dose vaccine series); AND have received the final dose of the COVID-19 vaccine at least 14 days prior to the hired employee’s first date of employment  Medical exemptions or any other kind of requested exemption based upon the Hospital’s obligations pursuant to the Ontario Human Rights Code will be considered on a case-by-case basis    Why Humber?
