Qualifications:

We're looking for a Peri-Operative Unit Aide to work in our Pre-Operative, SDC, and PACU units within the Surgical Program.
Reporting to the Manager, Clinical Programs – Surgical Daycare & Pre-Op:
Responsibilities:
- Ensuring that the areas and Supply carts are stocked, replacing linen, supplies, etc.
- Transporting patients within & to Surgical Services
- Other stocking duties as assigned.
- Transporting clean/soiled linen, medical surgical /supplies, patients, patient belongings specimens etc.
- Ensuring that all empty Bays are equipped with a clean stretcher
- Assisting pre-op patients with way finding to pre-operative change rooms
- Working with the volunteers in navigating patients in the pre-operative area
Requirements:
- Grade 12 education or equivalent.
- Two years of recent related hospital medical/surgical supply experience required.
- Medical Device Reprocessing (MDRD) or Sterile Supply Processing certificate preferred.

Our Critical Care Unit offers state of the art care for cardiac, medical and surgical critically ill patients.

Responsibilities:
• The Medical Program Social worker is required to be an active member of the Bariatric Programs Patient and Family Advisory Committee
• Responsible for overseeing and developing the program's peer support program
• Facilitate educational and support group classes Provide psychosocial assessment, crisis intervention, discharge planning, and counseling to patients/families.
• Act as liaison, assist patients and staff with community resources and facilitate their effective use.
• Working collaboratively with discharge planning and patient flow across the patient care continuum.
• Act as liaison with other programs, departments and patients to access community resources.
• Works as a member of the health care team.
• Actively participates in discussion and decision making with the appropriate inter-professional team.

Requirements:
• MSW obtained from an accredited School of Social Work required.

Position Responsibilities:
- Performing interventions focused on the following areas: splint/cast and dressings/bandages application; windowing/wedging casts; trimming, bi-valving casts, bandages and splints; removal/repair/re-enforce casts; removal of sutures/wires/pins/external fixators; application of principles of surgical asepsis; preparing for minor surgical procedures application; setting up surgical field for injections and minor procedures; and adjusting and removing orthopaedic devices.
- Assisting physicians on inpatient units and the plastics clinic with orthopedic procedures, including wound care, aspirations, minor surgical procedures and fracture reductions.
Performing/Participating in a comprehensive assessment of patients’ conditions, and develop an orthopedic care plan in collaboration with the family and the interdisciplinary health care team.

Qualifications:
- Graduate of a recognized Orthopaedic Technologist Programme sanctioned by the CSOT, apprenticeship programme, and sponsorship by an Orthopaedic Physician

- Providing appropriate assessment and treatment of referred patients with medical, cardiorespiratory, and orthopedic/surgical conditions within the surgical and inpatient Units.
- Prioritizing patient caseload to meet the needs of referred patients.
- Recording client assessment, progress and discharge status.
- Recording timely workload measurement data.
- Works as a member of the health care team.
- Flexibility in caseload and program assignment.
- Able to manage patient caseload in a busy acute care setting.
- Clinical supervision of Rehabilitation Assistants with the Ontario College of Physiotherapy support personnel standards.

Requirements:
- Bachelors or Master’s Degree in Physiotherapy.
- Current registration in good standing with the College of Physiotherapists of Ontario.

Requirements:
- Current RN Registration with the College of Nurses of Ontario required
- Masters of Nursing required
- BScN required
- Current BCLS and Non-Violent Crisis Intervention (CPI) certification required
- Minimum of 5 years recent experience in psychiatric nursing required
- Certification in Psychiatric Mental Health Nursing required
- Solid knowledge/experience of addictions an asset
- Solid knowledge/experience in Trauma Informed Care preferred
- Solid knowledge/experience in Child Adolescent Mental Health preferred
- Previous experience in an adult educator/Clinical Practice Leader role
- Experience in adult education principles that address facets of learning needs assessment
- Experience in the design, delivery and evaluation of educational programs
- Excellent verbal, written and computer skills

- Plans, coordinates and promotes the implementation of the innovation initiative intake, prioritization, resourcing, tracking, and reporting process and governance including project plans, timeframes, schedules, budgets and communication materials.
- Supports program leadership to develop governance committees’ terms of reference.
- Prepares for and coordinates committees’ meetings
- Coordinates the collection and compiles data at the direction of the program leadership including but not limited to: initiative requests, initiative disposition, project delivery metrics etc.
- Monitor and track intake progress and addresses issues that arise appropriately.
- Report and escalate potential or actual delays or barriers to project completion.
- Collaborates with other departments to communicate and educate the organization on the initiative intake process and other processes

- Microsoft 365 Word, Excel, Outlook, PowerPoint required, Medical terminology is preferred.
- Business and Statistical analysis skill required
- Experience monitoring budgets.

Responsibilities:
- Advocate for use of best practices for clinical staff.
- Guide clinical staff in the interpretation and implementation of professional and practice standards.
- Monitor and evaluate staff and program compliance with professional best practices and standards to identify and resolve issues with the delivery of quality, safe patient care and compliance with professional standards; refer complex issues to Director for resolution; adjust practice changes as they occur to bring staff to new standard.
- Develop reports of staff disciplinary actions for submission to regulatory health professional colleges and professional associations in collaboration with Manager and Director.
- Develop project plans, requirements, testing and implementation plans, resources and interdependencies, monitor implementation.
- Collect data on practice needs (e.g. programs, care interventions, policies, standards in clinical area) to identify gaps in nursing practices, competencies, and service quality; develop educational strategies, plans, and content (e.g. policies, processes, technologies) in consultation with unit leadership team and clinical staff; and evaluate post-training outcomes.
- Conduct impact analysis to determine training and education activity and program interdependencies (e.g. technical Meditech systems use, orientation, accreditation, hand hygiene, new or updated care and treatment standards) and keep staff updated with new practices, skills, and certifications in assigned clinical areas.
- Coach/mentor clinical staff; train new staff during orientation and monitor against best practice standards; recommend disciplinary actions for staff noncompliance with professional and operational requirements and standards to Director and monitor outcomes.

Requirements:
- Master’s Degree in Nursing or MSc in Nursing or equivalent combination of education and experience
- Current registration and membership in good standing with the College of Nurses of Ontario (CNO)
- Membership with the Registered Nurses Association of Ontario
- A minimum of (5) five years of relevant nursing experience in acute care, medical/surgical in a community based hospital
- Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications
- Demonstrated experience in Adult education to design, deliver and evaluate educational programs

Currently we have an exciting opportunity for a Registered Nurse to work in our Home Hemodialysis Unit, Nephrology Program. The Registered Nurse is responsible for addressing the needs of a unique patient population.
Using a Case Managed approach, the Registered Nurse coordinates clinical activities, acts as a clinical resource/role model for other staff members, and works directly with patients and members of the team to ensure timely and appropriate treatment and necessary follow up. The Nurse will autonomously assess and educate patients/family members/PSWs on home therapies ensuring that they have the necessary knowledge and skills to perform dialysis independently within their home. The nurse will also participate in a rotational on-call support for the program. This individual will closely liaise with the multidisciplinary team.
The Nurse working in the Home Hemodialysis Program will take a comprehensive approach to providing care for patients which chronic kidney disease wishing to dialyze independently or with assistance at home. The role will include assessment of patients being referred, visiting and assessing the home environment, training and ongoing primary case management.
As an integral team member of the Nephrology Program, there is an opportunity to participate within the Program by utilizing a chronic disease prevention and management (CDPM) and self-care philosophy, as the Program continues to achieve the deliverables under the Ontario Renal Network (ORN) strategic plan. This includes championing initiatives such as Person Centered Decision Making (PCD), Access to Kidney Transplantation (AKT) and other relevant and patient centered priorities.

Responsibilities:
- Performing all aspects of patient care including care paths, patient education, home and community visits, annual patient recertification, transplant workup, and monitoring quality indicators to ensure treatment adequacy and patient safety is maintained for all primary patients and as assigned
- Demonstrating a commitment to ongoing professional development within an evolving field
- Providing assessment, education, training and decision support for patients/family as it relates to independent dialysis
- Coordinating/facilitating patient appointments including clinic visits in collaboration with clerical team members
- Engaging the interdisciplinary team in a collaborative approach to professional practice issues and acts in the role of Liaison between all members who participate in the care of the patient and their family

Requirements:
- Registered and in good standing with the College of Nurses of Ontario
- BLS Provider certified required
- CNeph Certification preferred. Required to complete CNeph certification or equivalent within 28 months from the hiring date.
- Baccalaureate degree preferred
- Minimum of 2 years recent nephrology experience with at least 18 months in hemodialysis required
- Previous experience in pre-dialysis, transplantation, or home hemodialysis is an advantage
- Experience with ORN data collection preferred
- Proficiency in using Microsoft office (Word, Outlook, Power Point and Excel) and Meditech
- Demonstrated strong ability to assess a patient’s readiness to learn and apply the principles of adult learning while teaching patients and families

Right now, we are looking for an Executive Assistant to provide executive level, confidential administrative support to both the Vice President, of Post Acute Care & Health System Partnerships and the Chief Information Officer and serve as the primary point of contact for both areas.
The Executive Assistant to the Vice President, of Post Acute Care & Health System Partnerships and the Chief Information Officer will provide operational oversight of two portfolios including the Post Acute Care & Health System Partnerships and the Chief Information Officer's Portfolio. They will perform a wide range of administrative and operational support functions for day-to-day operations in the assigned executive office; coordinate day-to-day functions and related projects within their immediate office.
This individual will be responsible for providing confidential and advanced administrative duties supporting the business agendas for the Vice President, of Post Acute Care & Health System Partnerships and the Chief Information Officer and will interact with multiple internal and external stakeholders, upholding the highest standard of professionalism and confidentiality.

Reporting Relationship: Vice President, of Post Acute Care & Health System Partnerships and the Chief Information Officer
Key Responsibilities (but not limited to):
- Provide primary administrative support to Vice President Strategy and External Relations (VP S&ER) and the Chief Information Officer (CIO) and secondary administrative support to assigned staff within their reporting structures; exercise confidentiality in the management and release of all information; participate in project assignments within assigned Senior Management Team (SMT) reporting structure
- Act as the assigned office’s first point of contact for internal and external contacts to facilitate smooth, effective, and accurate information flows; balance priorities; respond to issues brought forward by leaders, staff, patients, patient families, volunteers, etc.; de-escalate complaints and refer to leadership staff for resolution
- Liaise and collaborate with executives and other executive and program assistants to communicate information, and coordinate / participate in cross-functional projects
- Research/investigate topics and develop summaries / reports /briefing notes/presentations using internal and external resources.
- Monitor and maintain office correspondence; draft responses to general issues; coordinate resources to support the executives in addressing identified issues; compose or edit a variety of highly confidential documents
- Maintain corporate manuals, policies, and procedures, and produce documents for various audiences.
- Research data sources; prepare summary documentation to support ideas, departmental projects and initiatives; access and review corporate systems information to analyze data, and compile a variety of statistical and / or qualitative reports
- Coordinate and monitor calendars, meeting logistics, and special event logistics.
- Arrange corporate travel to conferences and other events in compliance with corporate travel requirements and processes
- Develop and maintain organized filing systems; maintain records; locate files, documents, and other information
- Support assigned projects within assigned reporting structure; track project timelines against targets
- Track and monitor reporting and other deadlines required to meet reporting requirements
- Other administrative duties assigned.
Qualifications:
- High school graduate or equivalent.
- Completion of a Community College Office Administration Diploma (preferably Medical) or equivalent required, health administration focus preferred.
- Undergraduate degree in business management preferred.
- A minimum of (5) five years providing administrative and executive support required, in a healthcare/hospital environment preferred.
- Knowledge of medical and information technology terminology is an asset.
- Demonstrated excellent computer skills required, including Microsoft Word, Excel, Power Point, Zoom, Microsoft Team, and Outlook
- Good organizational and problem solving skills required, including an ability to prioritize a varied and busy workload. Demonstrated ability to cope with tight deadlines and frequent interruptions.
- Good keyboarding skills with a minimum typing speed of 60 wpm.
- Exceptional analytical and information-seeking skills that contribute to effective decision-making and strategic planning and execution of action plans that deliver results and motivate individuals for greater performance excellence.
- Proven business acumen and financial management.
- Self-directed, courageous, and highly motivated with excellent interpersonal and communication skills.
- Demonstrated track record as a highly regarded contributor within a management team
- Exceptional interpersonal, facilitation and negotiation skills.
- Knowledge and ability to facilitate meetings, conduct effective briefings and presentations both internally and externally with stakeholders who reflect the full range of diverse objectives.
