We currently have an exciting opportunity for a Child and Youth Worker to join our Mental Health & Addictions team to provide temporary system navigation and counselling support to patients and their families to successfully re-integrate into the community.
Hours of work: Must be able to work Monday thru Friday 8 hour days and evenings.
Hourly Rate Range: $31.64 - $37.89

Responsibilities:
- Assists children and youth up to 19 years of age with mental health and addiction challenges to transition from the hospital to the community
- Accepts referrals that meet eligibility criteria from the inpatient, outpatient, and urgent care clinic programs
- Engages clients and families by quickly establishing therapeutic rapport
- Identifies and coordinates community services to support patients and families’ mental health needs within their daily environment including school, home, employment, legal, and other community domains.
- Utilizes strong case management and advocacy skills, with extensive knowledge of community mental health resources with the Northwest OHT
- Possesses advanced clinical skills with children, youth and families with expertise in cognitive behavioural therapy (CBT), solution focused brief therapy, motivational interviewing
- Implements and evaluates behavioral management strategies as determined by the treatment team
- Prioritizes demands effectively to balance the complex needs of patients and families
- Facilitates the initial meeting with the patient and new service provider
- Assess and documents ongoing assessment of mental status, and communicates findings with community partners
- Ability to plan and lead psychoeducational therapeutic groups
- Conducts risk assessments in the community
- Providing cultural specific interventions
- Understands the impact of trauma, stigma within the context of culturally sensitive care
- Communicates consistently with other mental health professionals including psychiatrists, psychologist, child and youth workers, social workers, teachers, and nurses.
- Approaches patient care with a culturally sensitive, stigma-free and trauma informed lens
Requirements:
- Minimum of 5 year+ of recent Mental Health experience required.
- Diploma in Child and Youth Work or equivalent from recognized college required.
- Working knowledge of the Mental Health Act, Family Services Act, Criminal Youth Justice Act
- Advanced understanding of psychiatric illnesses and treatments, specifically mood/anxiety disorders, adjustment disorder, psychosis, parent/child related issues
- An awareness and Knowledge/experience in Trauma Informed Care preferred
- Understanding of assessment and treatment needs of a pediatric psychiatric population
- Excellent assessment skills.
- Good verbal and written communication skills.
- Ability to work within a multidisciplinary team as well as independently
- CPR or BCLS certification.
- A second language is an asset. Familiarity with cultural groups in catchment area and familiarity with community resources are assets.

As a member of the General Internal Medicine Team, you will help provide comprehensive medical care for patients from admission to discharge. In addition to clinical duties, each General Internist also has a role with respect to utilization management, clinical guideline implementation, program growth and participation. You will also take first call to the Emergency Department, and participate in a number of Out-patient Clinics.

Qualifications:
- Bachelors or Masters Degree in Food and Nutrition.
- Current registration in good standing with the College of Dietitians of Ontario.
- Completion of an internship program and a member (or eligibility to become a member) of the Dietitians of Canada.
- 1-2 years clinical experience is preferred.
- Experience in TPN an asset
- Excellent knowledge of clinical nutrition principles and evidence based nutrition practice
- Demonstrated excellent written and verbal communication skills, including an ability to encourage, support and motivate patients and/or their families from a multi-cultural community.
- Excellent time management and organizational skills.
- Able to work effectively in an interdisciplinary team.
- Ability to travel for cross-site coverage may be required. Access to a vehicle is an asset.

- Completes patient documentation, workload measurements and data requirements in accordance with Hospital, Ministry of Health and departmental standards.
- Acts as resource of discipline-specific expertise to other members of the team.
- Works in a manner to ensure safety to oneself, patients, colleagues and the work environment.

Qualifications:
- MSW obtained from an accredited School of Social Work.
- Current membership with OASW and OCSWSSW.

- Demonstrated skills in working with a broad range of patient groups and their families.

Responsibilities:
- Assessment and treatment of patients in the Outpatient Mental Health Service, with special emphasis on those individuals with serious and persistent mental illness.
- Participate in clinic activities, including the depot medication injection clinic and clozapine clinic if required.
- Collaborate with the Physician Leader, Manager and other stakeholders in the development of new programs for patients and families.
- Deliver individual/group therapy and psycho-education for patients and/or their families.
- Evaluate and perform psychiatric and physical status history/assessment of patients who are referred to the program and provide follow-up as required.
- Facilitate strong working relationship within our internal Mental Health and Addictions programs as well as within our community partners.
- Nurses may be reassigned to other mental health units as per the Collective Agreement
Requirements:
- Current registration with CNO required. Current BCLS certification required.
- BScN or degree in health sciences preferred.
- Certificate in Psychiatric Mental Health Nursing required or willingness to obtain when eligible to do so.
- Two years’ experience in Mental Health required; with two years’ experience in working with patients who have serious and persistent mental illness preferred.
- Excellent assessment skills using a bio-psycho-social model required.
- Experience/trained in providing therapy to patients (e.g. interpersonal, cognitive-behavioural, dynamic) is a strong asset.
- Excellent familiarity with psychotropic medications and ability to administer depot medication.
- An awareness and Knowledge/experience in Trauma Informed Care preferred
- Experience with addictions care is an asset, working knowledge of the Mental Health Act.
- Familiarity with the interplay between substance use history and mental health symptoms.
- Demonstrates strong initiative and possesses excellent organizational skills.
- Excellent interpersonal and communication skills and demonstrated ability to work autonomously as well as a part of the interdisciplinary team.

Position Responsibilities:
- Ensuring that the areas and Supply carts are stocked, replacing linen, isolation supplies, etc.
- Maintaining an established inventory of sterile and unsterile supplies for the units.
- Assiting with collection of urgent supplies and medications from pharmacy or equipment as needed.
- Other stocking duties as assigned.
- Contacting SPD, EVS and or Stores for additional supplies if necessary
- Transporting clean/soiled linen, medical surgical /supplies, specimens etc.
- Ensuring that all empty Bays are equipped with a clean stretcher
- Assisting surgical and plastic clinic patients with way finding to pre-operative change rooms
- Notifying manager/designate about broken equipment repairs or replacement and initiating the reporting and transportation of broken equipment to Biomedical Engineering or Maintenance as required

Right now we are looking for a Registered Nurse to work with our Haemodialysis Unit and be part of our Long Term Care Expansion Project.
Position Responsibilities:
- All aspects of care for patients receiving hemodialysis in the dialysis unit, long-term care unit, or congregate facilities partnered with HRH
- Provision of acute hemodialysis treatment on inpatient units, ICU, ER, and LTC partners
- Participation in on call rotation for provision of acute hemodialysis treatment
- Providing patient education and follow-up to episodic concerns raised during the dialysis treatment
- Providing comprehensive care to assigned patient that includes but not limited to ensuring plan of care is developed in collaboration with multidisciplinary team, updated and communicated with team in a timely manner
Qualifications:
- Current RN registration with the College of Nurses of Ontario

We have an exciting opportunity for a Full Time SCOPE/LTC+ Navigator to join our team within Integrated Health Systems & Partnerships at Humber River Hospital. This department works collaboratively with internal departments, community partners, primary care, agencies, Ontario Health and the Ministry of Health to plan and build an Integrated Health System to improve the health of our community. SCOPE (Seamless Care Optimization the Patient Experience) is a quality improvement collaboration between the hospital and primary care providers (PCPs) located in North West Toronto. The SCOPE service is a virtual inter-professional team that facilitates greater access to hospital-based interdisciplinary resources and other specialists care. SCOPEaims to reduce avoidable ED visits and hospital re-admissions for patients in need. LTC+ (Long Term Care +) is a virtual care model that provides direct access to a suite of services including real-time access to virtual GIM consultations, specialist care and community resources for the partner LTC homes in North West Toronto. LTC+ aims to deliver the best care possible to residents in LTC homes to avoid unnecessary transfers to hospital and improve access to outpatient clinics/resources.
Reporting to the Manager, Family Medicine Teaching Unit and Integrated Care Services , the SCOPE/LTC+ Navigator will coordinate and assist physicians in primary care and in Long Term Care Homes in navigating SCOPEservices, providing centralized access to hospital and community services, with follow-up on status of referrals, results, and consults. This role supports timely access, ensures navigator to appropriate resources, and reduced ED visits as patients are proactively connected to the right level of care

RESPONSIBLITIES
- Play a leadership role in the implementation, monitoring, and evaluation of the SCOPE and LTC+ program
- Participate in the implementation, monitoring, and evaluation of the SCOPE/LTC+ program
- Participates in internal and external committees/working groups
- Enhance work-flow and communication pathways between hospital, primary care and community care by identifying barriers to care and work towards creating new care pathways to promote and optimize seamless care for the client.
- Advocate for, and contribute to, the establishment of organizational structures and resources to support the growth of SCOPE and LTC+ in keeping with the direction and priorities of the organization
- Advocate to identify gaps in services, needed for individualized program consideration and system level changes required to meet changing needs of the patient population
- Develop and foster links with external partners to facilitate continuity of patient care; including community outreach
- Respond to all calls and emails made to SCOPE/LTC+ Navigation Hub by the PCPs, LTC physicians and Nurse Practitioners and their administrative assistants
- Assist Primary Care Providers in the community and LTC with system navigation, referrals to specialists, and resources in the community, and other appropriate community, clinical and specialized supports to address patient health concerns.
- Ensure seamless navigation of existing services; triage and forward referrals to appropriate SCOPE/LTC+ contact (i.e. Internist, Home and Community Care Coordinator, Imaging, Outpatient Clinics)
- Collaborate with the health care team to improve patient care as they transition through the health care system
- Collect relevant patient information required for assessment and database entry (Patient Name, DOB, HC#)
- Understand and clarify PCP primary concerns and request for support
- Work collaboratively and in relationship with the inter-professional team while role modelling relationship centered care
- Use concepts from complexity thinking and relational inquiry when in relationship with others.
- Collect Data for research evaluation for each referral – Database Entry forms to be completed with each referral and submitted to the evaluation team on a weekly basis
- Review available/relevant information from Meditech (EMR) to determine assessment and care planning
- If client is an existing or former clinic patient, will re-connect with staff to communicate PCP concerns and request for assessment
- This includes the clarification of referral criteria and identification of the resource that will best suit the health care need
- Improve communication and collaboration between consultants and primary care services by assisting PCP staff in the compilation of referral packages and follow-up of referrals
- Provide PCP or PCP staff with clinic/referral information
- Navigate hospital clinics and resources that will assist PCP with managing client medical, functional, cognitive, psychosocial conditions
- Direct requests for Internal medicine consultations or medical work-ups to SCOPE/LTC+’s Internist-On-Call
- Coordinate care delivery and the development of individualized care plans with the persons internal and external stakeholders including liaising with outside agencies to facilitate seamless care for persons across sectors – primary, acute and community care
REQUIREMENTS
- Bachelor of Science in Nursing
- Masters in Nursing or a related discipline
- Current registration and membership in good standing with the College of Nurses of Ontario
- Minimum of 3 years of experience within acute care and/or primary/community care
- Written and verbal communications with ability to explain and provide necessary information to patients, family members, physician and/or any stakeholders
- Experience working within an interprofessional team environment as well as independently
- Experience working with older patients with complex needs
- Demonstrated computer skills using MS Office (Outlook, Word, Excel, PowerPoint, Visio, etc.)
- Database management skills preferred with strong written and oral interpersonal skills
- Understand and appreciate primary care challenges

Our cardiology program provides specialty services for patients with acute and chronic conditions of the cardiovascular system. The Inpatient Cardiology Units are located on the 7th floor of our new site. Reporting to the Manager, Occupational Health Safety and Wellness, the Occupational Health Nurse (OHN) contributes to a safe and healthy work environment by developing, implementing and maintaining comprehensive health & safety programs for all those employed by/working at Humber River Hospital including staff, auxiliary, volunteers, physicians, and students.
In addition, the OHN provides oversight to Occupational Heath, Safety and Wellness Clinic, leads and oversees workplace health, safety and wellness; co-ordinates the case management of illness and injury claims, co-ordinates the employee return to work process, co-ordinates accommodation for those at work or returning to work and provides expertise on attendance support programs. The OHN assumes responsibility for providing the Employee Health Surveillance Program in accordance with the Public Hospitals Act, using the OHA/OMA Guidelines.
The OHN provides diverse healthcare services including first aid to prevent illness, maintain health/wellness, and provides early intervention and rehabilitation when injuries or health problems occur. The OHN advises management and employees on Occupational Health issues in the workplace and assists to ensure the organization is in regulatory compliance.

Responsibilities (include but are not limited to):
- Provide safety, disability management, and nursing (Occupational Health) services for all staff and volunteers
- To develop, implement, and maintain comprehensive health & safety programs that contribute to a safe and healthy work environment for all employees and workers (e.g. staff, volunteers, physicians, and students)
- Deliver diverse healthcare and/or first aid services to prevent illness , maintain health/wellness
- Perform clinical assessments and provide educational support to employees; triage referrals to appropriate internal and external physicians and other health care providers; provide employee health information to external health partners
- Coordinate case management activities for employee illness and injury claims including return to work processes
- Provide early intervention and rehabilitation when injuries or health problems occur
- Coordinate workplace accommodations for those at work or returning to work
- Support to Unit Managers and HR regarding the attendance program
- Coordinate the Employee Health Surveillance Program in accordance with the Public Hospitals Act, using the OHA/OMA Guidelines
- Advise management and employees on Occupational Health issues to ensure organizational compliance with regulatory requirements
- Market, promote, and maintain proactive wellness initiatives and education materials and programs on a variety of health, safety, and wellness topics that suit a broad range of patient, employer, public, and WSIB needs
- Research, develop, design, and implement educational and wellness initiatives for multiple stakeholders
- Provide information and education for leaders and staff regarding OHS processes related to short-term and long-term disability (STD and LTD) claims
- Participate in needs monitoring, assessment, and implementation of Occupational Health Programs
- Conduct new hire health assessments, mask fit tests, immunization tests and surveillance, flu clinics, counselling, and debrief critical incidents
- Prepare, analyze, and distribute reports for Toronto Public Health and other external stakeholders to meet legal and regulatory requirements (e.g. employee immunization status, employee injury/illness trends, immunization statistics, etc.)
- Identify and integrate preventative measures to promote health and control health care hazards (e.g. biological, physical, chemical, ergonomic, psychological)
- Maintain first aid and other supplies required for the delivery of the OH program
Job requirements:
- Current registration in good standing with the College of Nurses of Ontario (RN required);
- Masters Degree in Nursing or related field preferred
- Bachelor of Science (Nursing) required
- Certified Occupational Health Nurse Designation (COHN) preferred (or commitment to complete within first 2 years of employment)
- Occupational Health Nurse experience preferred
- 3-5 years acute care nursing experience required
- Current OOHNA membership preferred;
- Current Standard First Aid Certificate required
- Experience in Disability Management and Occupational health field managing WSIB, STD, LTD and Rehabilitation claims management - preferred
- Strong computer skills in MS Office required, Parklane system preferred; expertise in relevant legislation, e.g. WSIA, Occupational Health & Safety Act, Human Rights Code;
- Demonstrated skills in planning & delivering education / training programs to adults;
- Demonstrated ability to deal effectively with employees at all levels;
- Demonstrated project planning, analytical, problem-solving, judgment, time management, negotiation and conflict resolution skills;
- Ability to work independently while maintaining collaborative relationships with internal & external contacts;
- Excellent presentation, verbal and written communication skills;
- Strong customer service focus;
- Professional attitude that demonstrates results oriented and opened to a changing environment
- Excellent attendance and discipline free record required.
