As part of our dynamic team, you can lead the way, as we continue our journey towards high reliability care!   Under the direction of the Director, Surgical Programs, the Orthopaedic Care Navigator is responsible for coordinating and assisting patients in navigating orthopaedic care within the hospital, their family doctor, and community healthcare centres and for facilitating positive patient experience across their entire continuum of care from pre-op to their post-acute phase. This role embraces the concept of seamless care for patients undergoing hip or knee joint replacement and other orthopaedic surgical procedures.    We currently have an exciting opportunity for a Orthopaedic Care Navigator, for SURGICAL In-Patients & Clinics.   Reporting Relationship: Program Director, Surgery Hours of work: 37.5 hours per week (some irregularity in hours may be required) Employee Group: Non-union   Location: Humber River Health - Wilson POSITION RESPONSIBILITIES (included but not limited to): - Program Design and Development - Act as a member of a specialized interdisciplinary orthopaedic care program in the Surgical In-patient and Clinics portfolio to support assessment, follow-up, and treatment referral processes aligned with clinical best practice guidelines and evidence-based research - Remain abreast of new developments in orthopaedic care related evidence-based programs to disseminate and/or utilize developments and new knowledge into clinical practice; integrate research into practice to ensure best practice standards are maintained - Participate in the implementation, monitoring, and evaluation of standards specific to orthopaedic care assessment and health follow up - Conduct frequent environmental scans to identify gaps in clinical delivery for opportunities for improvement and promote ongoing quality assurance; and plan, implement, evaluate, and report on outcomes to all stakeholders, including mapping present and future trends, to create and sustain a seamless and continuous process for patients undergoing elective orthopaedic surgery - Influence clinical practice and patient outcomes while collaborating with the broader health care team and community partners regarding complex and challenging patient care situations - Participate in the collection and evaluation of health care data required that maybe required by any stakeholders, including the MOHLTC - Develop and foster links with external partners to facilitate continuity of patient care   - Patient Care Guidance - Supports the patient through the available treatment options to develop health care plans, and along the entire episode of care: prevention, diagnosis, preparation/plan, treatment, recovery/rehabilitation and monitoring/management to ensure that every patient receives care that is coordinated and navigated along the patient care path and/or entire process (preoperatively, postoperatively, recovery and rehabilitation) - Liaise with patient primary care teams (e.g. physicians, orthopaedic specialists, radiologists, surgeons, primary health practitioners, and community agencies) to facilitate the delivery of patient care services - Provide supportive care resources to patients and families to address psychosocial, practical, and informational needs across their entire continuum of care from pre-op to their post-acute phase - Develop and deliver to patients/families and staff orthopaedic health educational sessions assessment, intervention and coordination within the care pathway QUALIFICATIONS: - Graduate degree in Nursing - 3 years of recent related experience in acute care and/or orthopaedic nursing including but not limited to those specifically outlined in the Responsibilities.  - Knowledge/Understanding: - Orthopaedic care pathways, evidence based programs, and clinical practice standards - Education principles - Evidence of program design, resources development, evaluation methodologies - Quality improvement processes, data analysis, program development - College of Nurses of Ontario standards of practice - Canadian Orthopaedic Nurses Association (CONA) standards - Accessibility for Ontarians with Disabilities Act (AODA), Occupational Health & Safety Act (OH&A), Workplace Hazardous Materials Information System (WHMIS) preferred - Freedom of Information and Personal Privacy Act (FIPPA), Personal Health Information Protection Act (PHIPA), Personal Information Protection and Electronic Documents Act (PIPEDA) preferred - Demonstrated skills/abilities: - Expert clinical skills in the assessment  planning, implementing, evaluating and documenting the delivery and coordination of care for the  hip and knee replacement patients and their families Community resources - Exceptional facilitation and influencing skills with ability to build relationship with patients and family members, physicians, and all stakeholders involved in the patient care - Excellent practice program development, project management, change management and report writing skills - Excellent leadership, collaboration, decision making, problem recognition and problem solving, and critical thinking skills - Self-directed and highly motivated with excellent interpersonal and communication skills - Written and verbal communications with ability to explain and provide necessary information to patients, family members, physician and/or any stakeholders  - Ability to work independently and within a team environment - Effective and efficient time management and organization skills with ability to manage competing demands in a fast-paced, multi-site, multi-disciplinary health care environment - High level of initiative and able to effectively lead project teams to achieve outcomes, preferred - Demonstrated and ongoing commitment to continuing professional development - Excellent attendance and discipline free record   Why choose Humber River Health?   At Humber River Health, our staff, physicians, and volunteers are lighting new ways in healthcare. Student Nurse Externs will be responsible for the provision of basic care (activities of daily living such as bathing, toileting, feeding, etc.) needs for up to 10 patients each. The Student Nurse Extern may work under the supervision of both Registered Nurses and Registered Practical Nurses in accordance to department policies and procedures. The Student Nurse Extern will also be responsible for the documentation of activities of daily living (ADLs), such as intake and output, and relevant communication for patients in which they have provided care for.
