Position Responsibilities:
- Patient population includes a variety of areas and may include inpatient acute medical, ICU, Surgical, Emergency and Geriatric Patients.
- Provide psychosocial assessment, crisis intervention, discharge planning and counselling to patients and their families.
- Act as liaison and assist patients and staff with community resources.
- Discharge planning and patient flow within the acute and community sectors.
- Works as a member of the health care team.
- Flexibility in caseload and program assignment.
- Able to manage and prioritize patient caseload in a busy acute care setting.
- Participates in reflective practice as per College standards.
- Clinical supervision of social work students.
- Provides cross-coverage of Social Work Services for vacation/staffing relief.
- Cross coverage may also include coverage at the Reactivation Care Centres (RCC) located at the Finch Site and Church Sites.
Qualifications:
- MSW obtained from an accredited school of Social Work required.
- Current membership with OASW and OCCSWSSW, or eligibility to become a member required.
- Experience in the acute and/or sub-acute sector system, patient flow and discharge planning is an asset and preferred.
- Computer skills required; ability to manage electronic recording, workload and meet submission timelines.
- Demonstrated knowledge of the acute, sub-acute and community sector. Knowledge of community resources an asset.
- Demonstrated organizational skills to set clinical and non-clinical priorities in a fast paced environment.

Reporting Relationship: Manager of Mental Health
Hourly Rate Range: $34.24 - $49.02
Schedule Requirements: Day shift Monday to Friday, some evenings may be required
Location: Wilson Site - Must be willing to work on all Mental Health Units

Position Responsibilities:
- Process and review external/internal mental health referrals directed to the Outpatient Mental Health program
- Liaise with referral sources for clarification of referral and provide instructions regarding referral criteria to ensure complete and appropriate information is received
- Conduct intake screener assessments (via telephone, video or in person) to clarify needs of patient and offer services best suited for the referral
- Conduct risk assessments and interventions when clinically indicated
- Provide patients with relevant mental health and addictions community resources
- Deliver patient educational groups (via video or in person) for referred patients
- Present complex referrals/assessments in team clinical rounds and communicate decisions with referral source and patient as appropriate
- Participate in department related projects or committees to improve intake processes
- Liaise with internal mental health team members to ensure correct placement of referrals
- Provides coverage to other areas of the Outpatient Mental Health program as needed.
Qualifications:
- Current registration with the College of Nurses of Ontario.
- Current BCLS/CPR certification required.
- BScN preferred.
- Certification in Psychiatric Mental Health Nursing required, or willingness to obtain within one year of hire.
- 2 years of recent mental health/addictions experience is required.
- Demonstrated ability to provide individual/group health teaching for patients and staff.
- Excellent assessment skills, including demonstrated knowledge of crisis intervention skills.
- Demonstrates awareness and understanding in working with individuals from different cultures.
- Demonstrated ability to work with complex patient and family situations
- Demonstrates an excellent understanding re: the etiology of mental health/addiction illnesses.
- Excellent knowledge of the Mental Health Act.
- Knowledge/experience in Trauma Informed Care preferred.
- Technology literacy and usage skills preferred. Such as: video conferencing using MS Teams, OTN etc, and ability to use mobile technologies such as smartphones and tablets to connect patients to care.
- Demonstrated knowledge of Mental Health & Addictions community resources is an asset

- Recording timely workload measurement data.
- Works as a member of the health care team.

Position Responsibilities:
- Care of acutely ill cardiac medical and surgical patients.
- Care of patients requiring Hemodynamic and ventilation monitoring by means of invasive and non- invasive approaches.
- Care planning and coordination of care with various team members embracing patient centered care.
- Utilization of computerized systems for documentation of patient care.
- Participation in project teams or Hospital committees as required.
Qualifications:
- Current RN registration with the College of Nurses of Ontario required.
- Current BCLS certification required; ACLS preferred.
- Minimum 2 years Critical Care experience.

- Experience in working on a Nurse Lead Rapid Response Team preferred

Position Responsibilities:
- Provides high quality administrative clerical support services to the Program Director and Program
- Attends and co-ordinates the preparation of meeting agendas, recording minutes and communications for committee meetings.
- Arranges appointments and meetings
- Travelling across sites to support meetings and orientation.
- Assists with coordination Program wide events (e.g. Clinical Day, Grand Rounds) and projects.
- Coordinates office functions including overseeing and monitoring the workflow of clerical staff.

- Assists with development, revision and maintenance of Pharmacy policies and procedures system.

Qualifications:
- Minimum five years of administrative/secretarial experience supporting management, including taking/preparing committee minutes (committee administrative support)
- Completion of a recognized secretarial/administrative program is required.
- Previous experience in a health care environment- Clinic, hospital and/or physician’s office
- Knowledge of medical terminology is an asset.
- Demonstrated excellent computer skills, including proficiency in Microsoft Office (Word, Access, Excel, and Power Point), with a typing speed of 60 W.P.M is required.

The Graduate Nursing Students Extern role is an unregulated healthcare worker providing direct personal support to HRH patients. We currently have an exciting opportunity for a General X-Ray Technologist to join our Medical Imaging team.
Responsibilities:
- Perform all x-ray exams as per determined departmental protocols.
- Includes general x-ray, O.R., Portables, Gastric exams, Fluoroscopy and minor procedures
- Ability to operate all types of equipment including general x-ray rooms (DR & CR), mobile x-ray, c-arm fluoroscopy, general fluoroscopy and multi-purpose rooms.
- Prepare and assess patients for radiologic procedures including obtaining consent and confirming exam validity, resourcing radiologists as is necessary and sterile technique practices.
- Complete exam process by image review, confirming documentation and presenting to radiologists for reporting via film and/or PACS.
- Knowledge of and dutifully follows radiation safety practices for the protection of the patient, colleagues and their own occupational dose.
Requirements:
- Must be registered and in good standing with CMRITO. CAMRT is preferred.

- Support the project/initiative prioritization process as appropriate (e.g., development of business case, QA of project proposals/estimates, analyzing feasibility and sequencing), ensuring transparency of the prioritization process and regular communication with all stakeholders.

Qualifications:
- 5 years related work experience on digital health related projects – primarily in hospital setting or in integrated teams.
- Minimum 3 years of demonstrated progressive Project Management experience – primarily in healthcare/hospital setting.
- Successful completion of post-secondary education in Computer Science, Health Informatics, or a related field.
- Project Management Professional (PMP) designation is required.
- Change management certification is preferred.
- Strong analytical and problem-solving skills, with a strategic approach to work. Develops in-depth knowledge of project.
- Proven ability to develop relationships with key stakeholders to establish trust, credibility and respect.
- Collaborative leadership style with the ability to work in a multi-disciplinary team environment.
- Excellent presentation, technical writing, facilitation and training skills.
- Ability to prioritize and execute tasks in a fast-paced environment.
- Ability to work on multiple projects and/or deliverables concurrently.
- Advanced knowledge of Microsoft Word, PowerPoint, Project, Visio and Excel, project management related software required.
