At Humber River Health Our Oncology team provides a full range of support to patients who are living with cancer, at all phases of their cancer experience - consultation, diagnosis, treatment, follow-up, and palliative and supportive care.
Right now we are seeking a Registered Nurse for our 13 West Oncology Unit.
Our Oncology nurses specialize in treating people with cancer serving as their first line of communication as they help to coordinate and deliver many aspects of a patient’s treatment, including administering medications, fluids and cancer treatments such as chemotherapy. They are caring individuals who are dedicated to assisting patients as they undergo treatment.

Position Responsibilities:
- Administration of Chemotherapy. The teaching and certification process is provided by the hospital

Responsibilities:
- Provision of Nurse Practitioner scope of clinical care for LTC or congregate setting residents in accordance with the College of Nurses (CNO) Practice Standard for Nurse Practitioners and all other applicable legislation, including assessment, ordering and interpreting diagnostics, prescribing pharmacological agents and therapeutic measures, and performing controlled acts
- Collaboration with hospital and LTC teams to assist in identifying potential barriers and supporting with discharge strategies to reduce avoidable ED visits, decrease LOS, and minimize readmissions
- Capacity building for LTC staff to develop and enhance competency for acute treatment protocols

Job Summary:
The Nurse Practitioner is a leadership role within a multi\disciplinary and inter-professional team. This Nurse Practitioner role in NLOT team is primarily responsible for providing clinical care to support Long Term Care (LTC) homes. or congregate settings. The NP works in collaboration and consultation with a physician, the LTC or congregate setting coordinators, and members of the inter-professional health care team to respond to facilitate hospital discharge, and prevent ED visits. The NP will act as a link between hospital and the LTC homes to ensure the most appropriate and efficient discharge.
Responsibilities:
- Provision of Nurse Practitioner scope of clinical care for LTC or congregate setting residents in accordance with the College of Nurses (CNO) Practice Standard for Nurse Practitioners and all other applicable legislation, including assessment, ordering and interpreting diagnostics, prescribing pharmacological agents and therapeutic measures, and performing controlled acts
- Collaboration with hospital and LTC teams to assist in identifying potential barriers and supporting with discharge strategies to reduce avoidable ED visits, decrease LOS, and minimize readmissions
- Capacity building for LTC staff to develop and enhance competency for acute treatment protocols
- Integration of best practice guidelines and current evidence-based research into clinical practice for a select patient population
- Evaluation of outcomes for the programs and make recommendations for change in consultation with Program Director and Manager
- Development of linkages with external partners as appropriate to ensure continuity of care
- Enhance communication between hospital and LTC to support the plan of care transitions in care facilities
- Addressing medication reconciliation issues upon discharge from hospital
- In keeping with professional standards of practice, obtains and/or disseminates only that information which is relevant to meeting the client’s identified needs and with appropriate legal authorization, either in the form of signed consents or direct authorization from the client or designated substitute decision-maker.
- Utilization of expert communication skills by being a link between the client, consultants, resources and the referral source which may be required to coordinate the provision of service to the client.
- Participate in meetings and committees to support the planning and implementation of best practices to support needs of the residents in LTC homes
- Participates in reflective practice as per identified college standards
Requirements:
- Master in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master's Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required
- 1-3 years of demonstrated experience as a Nurse Practitioner required
- Previous experience in LTC sector preferred
- 3-5 years of relevant Acute medicine or Primary Care experience
- CNA Certification in Geriatrics would be an asset
- BCLS required
- Additional specialty education in Gerontology preferred
- Demonstrated experience in Adult education an asset
- Clinical specialty organizational membership: RNAO (required), CANO or ONS also preferred

Job Summary:
The Nurse Practitioner is a leadership role within a multi\disciplinary and inter-professional team. This Nurse Practitioner role in NLOT team is primarily responsible for providing clinical care to support Long Term Care (LTC) homes. or congregate settings. The NP works in collaboration and consultation with a physician, the LTC or congregate setting coordinators, and members of the inter-professional health care team to respond to facilitate hospital discharge, and prevent ED visits. The NP will act as a link between hospital and the LTC homes to ensure the most appropriate and efficient discharge.
Responsibilities:
- Provision of Nurse Practitioner scope of clinical care for LTC or congregate setting residents in accordance with the College of Nurses (CNO) Practice Standard for Nurse Practitioners and all other applicable legislation, including assessment, ordering and interpreting diagnostics, prescribing pharmacological agents and therapeutic measures, and performing controlled acts
- Collaboration with hospital and LTC teams to assist in identifying potential barriers and supporting with discharge strategies to reduce avoidable ED visits, decrease LOS, and minimize readmissions
- Capacity building for LTC staff to develop and enhance competency for acute treatment protocols
- Integration of best practice guidelines and current evidence-based research into clinical practice for a select patient population
- Evaluation of outcomes for the programs and make recommendations for change in consultation with Program Director and Manager
- Development of linkages with external partners as appropriate to ensure continuity of care

Requirements:

- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required

Our cardiology program provides specialty services for patients with acute and chronic conditions of the cardiovascular system. The Inpatient Cardiology Units are located on the 7th floor of our new site. The Student Nurse Extern role is an unregulated healthcare worker providing direct personal support to HRH patients. Student Nurse Externs will be responsible for the provision of basic care (activities of daily living such as bathing, toileting, feeding, etc.) needs for 4-5 Mother and baby)patients each. The Student Nurse Extern may work under the supervision of Registered Nurses in accordance to department policies and procedures. Reporting Relationship: Clerical Supervisor, Emergency Department
Hiring Salary Range: $25.993 - $26.904
Location: Humber River Health – Wilson Site
Hours of work: 8 hour days/evenings/nights and continuous weekend work
Union: Teamsters Clerical
Position Responsibilities:
- Responsible for clerical support including patient registration and discharge, maintenance of patient records, order entry and statistics.
- Responsible for customer service including scheduling appointments, answering patient inquiries, telephone inquiries, greeting patients and visitors in a courteous manner.
- Communicates with bed allocation and other departments to arrange patient placement.
- Communicates with other departments and service areas
Qualifications:
- High school graduate or equivalent.
- Completion of a Community College Medical Secretary Diploma an asset.
- Completion of a medical terminology course preferred.
- Minimum 1 year full time equivalent recent clerical experience required, preferably in a health care setting.
- Recent Emergency registration experience preferred, including EDM.
- Good keyboarding skills, with minimum typing speed of 40 WPM.
- Computer proficiency including Microsoft Office Suite.
- Strong interpersonal skills, both oral and written
- MEDITECH computer experience an asset
- Good organizational and problem solving skills required, including an ability to prioritize a varied and busy workload. Demonstrated ability to cope with tight deadlines, frequent interruptions and emergency situations
- Ability to work independently with minimal supervision in a team environment
- Knowledge of a second language an asset
- Demonstrated customer service skills.
- Applicants may be required to successfully complete full administrative testing, to include typing speed and MS Office.

Position Responsibilities:
The successful applicant will perform the duties expected of a Registered Nurse within an acute care hospital Emergency Department.
Qualifications:
- Current RN registration with the College of Nurses of Ontario in good standing
- Minimum 3-6 months medical/ surgical experience required; 6-8 weeks training provided
- Current Emergency experience preferred; ICU experience is an asset
- BCLS certification upon hire to show proof of valid current certification
- Coronary Care I & II certificates or the equivalent is preferred or completed within 1 year of your start date
- ACLS course upon hire or completed within six months of your start date required. Course available through Humber River Health
- TNCC, PALS or ENPC courses upon hire or completed within 1 year of your start date required.
- Excellent physical assessment skills
- Excellent interpersonal and communication skills with an ability to work effectively with a variety of staff, patients and families from diverse cultural and socioeconomic backgrounds
- Good patient/family teaching skills
