Right now we’re looking for a Registered Dietitian to work in our Bariatric Clinic. Humber River Hospital’s Surgical Program is recognized across Ontario and around the globe for its excellence in a broad range of surgical specialties and sub-specialties, for both pediatric and adult patients.
Responsibilities:
- As a member of the health care team, this position assumes the responsibility for assessing, monitoring, evaluating, teaching and providing nutrition recommendations and interventions for in and out-patients in the bariatric program.
- Ongoing development and revision of teaching aids and resource materials.
- Practice-based research and development of best practice/ evidence-based guidelines in dietetic practice in bariatric surgery
- Collaboration with multidisciplinary team members within and outside the organization
Requirements:
- Bachelor’s degree in Food and Nutrition.
- BSc. in Nutrition or related field preferred.
- Must be a member in good standing with the College of Dietitians of Ontario.
- Completion of an accredited Dietetic Internship program and a member (or eligibility to become a member) of the Dietitians of Canada
- Certified Bariatric Educator Designation preferred.
- Experience educating Bariatric patients strongly preferred.
- Experience teaching group classes strongly preferred.
- Clinical experience with acute medical/surgical patients preferred.
- Diabetes education or Bariatrics experience preferred.
- Demonstrated excellent written and verbal communication skills, including an ability to encourage, support and motivate patients and/or their families from a multi-cultural community
- Excellent presentation skills. with experience applying principles of adult education and the ability to teach in an individual or group setting.
- Excellent time management and organizational skills
- Excellent attendance and discipline free record required.

Position Responsibilities:
- Care of acutely ill cardiac medical and surgical patients.
- Care of patients requiring Hemodynamic and ventilation monitoring by means of invasive and non- invasive approaches.
- Care planning and coordination of care with various team members embracing patient centered care.
- Utilization of computerized systems for documentation of patient care.
- Participation in project teams or Hospital committees as required.
Qualifications:
- Current RN registration with the College of Nurses of Ontario required.
- Current BCLS certification required; ACLS preferred.
- Minimum 2 years Critical Care experience.

Our cardiology program provides specialty services for patients with acute and chronic conditions of the cardiovascular system. The successful candidate will be responsible for Registered Practical Nurse duties in all Medical and Surgical Inpatient Units. It is the expectation that members of the Nursing Resources Team are available for all rotating shifts.
Qualifications:
- Graduate from a recognized Registered Practical Nursing two year diploma program.
- Current RPN registration with the College of Nurses of Ontario.
- Current BCLS certification or demonstrated competency.
- Minimum 1 year acute medical/surgical experience, preferred.
- Minimum 2 years experience working with the Nurse Resource Team is an asset.

Position Responsibilities:
- RN responsibilities with a focus on assessment and total patient care to women in the antepartum, intrapartum and the immediate postpartum period as well as stabilization and transitional care of the newborn.
- Scrubbing and circulating for cesarean births.
- Patient and family education and support.
- Expectation to work on the Mother Baby Unit when the need arises.
Qualifications:
- Current RN registration with the College of Nurses of Ontario required.
- BCLS/CPR certification upon hire to show proof of valid current certification (Registered Nurses currently employed by HRH are exempted).
- Current NRP certificate required or agreement to obtain within 3 months of commencement into position.
- Fetal Monitoring Course required or agreement to obtain within 3 months of commencement into position.
- Recent Birthing Unit experience required.

- Proficiency in initiating IV therapy and venipuncture skills preferred.

Responsibilities:
- Acting as the first point of contact for all computer and related hardware, application and telecom related requests for IT service and support.
- Resolve technical issues related to servers, computers, printers, computer peripheral devices and telecommunication devices.
- Troubleshoot and resolve issues with Microsoft Windows operating system, Microsoft Office and health information applications.
- Recording, categorizing and logging requests for service using a central ticket management solution;
- Leveraging available tools (SMS, remote support utilities, knowledge base and other sources of documentation) to resolve incidents in an efficient and effective manner
- Transferring and escalating incidents and problems to 2nd/3rd level support resources and following up with ticket owners to ensure resolution in a timely manner;
- Provisioning, changing and de-provisioning user accounts in accordance with corporate policies and procedures
- Ability to provide training to staff and users.
- Develops procedures and provides documentation for staff and users manuals.
- Upgrading existing PC Hardware and software.
- Provide technical support to hospital staff during business and after hours by participating in on-call rotation
- Provides end-user support for the installation and support of applications.

Requirements:
- Successful completion of University and/or Community College diploma in a health-related or computer technology program.
- Minimum 3 years’ experience in a Technical Service Desk role is required.
- Minimum 3 years’ experience with expert level proficiency in Microsoft, Windows 7, Windows 8 and Microsoft Office 2013, MS Server 2008/12, MS SQL 2008/12, and Citrix Desktop is required.
- Experience with Meditech and other healthcare information applications is an asset.
- Experience with troubleshooting and resolving computer hardware, printer hardware, peripherals, and E-Mail Systems issues is required.
- Experience in troubleshooting Local and Wide area networks and is an asset.
- ITIL Foundation experiences and/or certification is an asset.
- A highly developed customer-service orientation.

This position provides support to a large Mental Health and Addictions program. It aims to support the program by providing:
- Clinical care in the form of psychological assessments and treatment (individual and group) – CBT is required.
- Program leadership around quality initiatives; including having an awareness of evidence-based standards/ best practices to help develop and evaluate programmatic changes.
- Provide specialized training and support for staff (e.g. consultation regarding CBT) and other in-service needs.
- Provide Student learning by selecting and supervising at least one appropriate student from affiliated university programs per academic year.
Responsibilities:
- In consultation with theMental Health interdisciplinary team, will plan and implement comprehensive psychological assessments using structured clinical interviews, behavioural, personality and cognitive/ neuropsychological measures; administer and interpret psychometric measures to assess behaviour, personality, social-emotional functioning, cognitive functioning and academic achievement; provide feedback and recommendations (both oral and comprehensive written reports ) to patients, families, and the interdisciplinary team for all assessments to clarify diagnosis, guide treatment;plan, implement and evaluate diverse empirically based interventions with adults; and participate in discharge planning and coordination of services within the hospital and in the community, as needed.
- Communicate and record patient notes, assessment results& progress reports in accordance with hospital policy and Standards of Professional Conduct.
- Consult with other staff members and or community professionals including developing, implementing and evaluating inpatient and or outpatient interventions implemented by the treatment team and other professionals within the patient’s circle of care.
- Provide in-service education.
- Supervise at least one practicum student per year from affiliated university programs.
- Provide consultation and training to staff with regard to the implementation of programmatic standardized assessment tools, CBT and iCBT.
- Participate in planning, preparation, facilitation and evaluation of new and existing programs and services within the program.
Requirements:
- Doctorate /Masters Ph.D in clinical psychology in area of Adult MH required (ability to work with Older Adults is an asset).
- Registration with the College of Psychologists of Ontario required
- Minimum of 5 years experience in Adult mental health
- Must have experience working with MS Office (Outlook, Word, Excel, PowerPoint, etc.)
- Skills in undertaking psychological assessments (including cognitive, diagnostic, and personality) required
- Training and experience with CBT required.
- An awareness and Knowledge/experience in Trauma Informed Care preferred
- Experience in Project Management, preferred
- Good knowledge of the Mental Healthcare

- Performs, as a member of a specialized interdisciplinary Cancer Care Program in the Breast Health Centre
- Helps guide patients in care planning and through all available treatment options
- Integrates best practice guidelines and evidence based research into clinical practice related to breast cancer population
- Works collaboratively with the interdisciplinary team to support the ongoing functioning of the breast health center related to assessment and follow up, and referrals for treatment
- Liaises with patient’s primary care physician, oncologist, radiologist, surgeon, primary health practitioners and other community agencies as required throughout the care process
- In alignment with Cancer Care Ontario’s breast cancer pathway, provides supportive care resources to patients and families to address psychosocial, practical and informational needs in the pre- diagnostic phase, during the diagnostic phase and beyond
- Keeps abreast of new developments in Disease Pathway Management and cancer related evidence-based programs, and disseminate into clinical practice
- Participates in the implementation, monitoring, and evaluation of standards specific to breast assessment and breast health follow up
- Participates in the collection of and evaluation of data as set out by Cancer Care Ontario and Ontario Breast Screening Program
- Conducts frequent environmental scans to identify gaps and areas for improvement, promoting ongoing quality assurance, plans, implements, evaluates and reports on outcomes to all stakeholders

Requirements:
- Master’s Degree in Nursing with clinical focus in Oncology

Position Responsibilities:
The successful applicant will perform the duties expected of a Registered Nurse within an acute care hospital Emergency Department.
Qualifications:
- Current RN registration with the College of Nurses of Ontario in good standing
- Minimum 3-6 months medical/ surgical experience required; 6-8 weeks training provided
- Current Emergency experience preferred; ICU experience is an asset
- BCLS certification upon hire to show proof of valid current certification
- Coronary Care I & II certificates or the equivalent is preferred or completed within 1 year of your start date
- ACLS course upon hire or completed within six months of your start date required. Course available through Humber River Hospital
- TNCC, PALS or ENPC courses upon hire or completed within 1 year of your start date required.
- Excellent physical assessment skills
- Excellent interpersonal and communication skills with an ability to work effectively with a variety of staff, patients and families from diverse cultural and socioeconomic backgrounds
- Good patient/family teaching skills

Qualifications:

- Minimum 2 years Critical Care experience.
- Critical Care Certificate required.
- Experience in working on a Nurse Lead Rapid Response Team preferred
- Experience in critical care with hemodynamic monitoring and invasive and non invasive ventilation required.

- Good problem solving skills required.

Reporting Relationship: Manager, Home Dialysis Program, Transition Care Unit, & Haemodialysis (Church Site)
Hiring Salary Range: $34.24 - $49.02
Hours of work: Site open 7 days per week, must be available to work Day/Evening 8 & 12 hour shifts

Position Responsibilities:
- All aspects of care for patients receiving hemodialysis with a primary nursing focus
- Providing education and follow-up to all hemodialysis patients
- Providing primary care to assigned patient that includes but not limited to ensuring plan of care is developed in collaboration with multidisciplinary team, updated and communicated with team in a timely manner
Qualifications:
- Current RN registration with the College of Nurses of Ontario
- BCLS certification upon hire to show proof of valid current certification (Registered Nurses currently employed by HRH are exempted).
- BScN preferred
- Minimum two year acute Medical / Surgical experience, required.
- Minimum two years recent Hemodialysis experience is required.
- Nephrology courses 1 to 4 completed or commitment to complete within an agreed period of time.
- Certification in Nephrology an asset or commitment to complete within an agreed period of time
- Patient assessment course and demonstrated good patient assessment skills
- Experience in peritoneal dialysis an asset
- Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients, family members and visitors
- Demonstrated good patient/family teaching skills
- Ability to prioritize and complete assignments in a reasonable length of time:
- Demonstrated sound judgement, good observation and patient assessment skills, tact and empathy;
- Demonstrated ability to perform appropriately in emergency situations;
- Proven ability to perform a leadership/preceptorship role;
- Physically capable of performing the essential duties of the position;
- Demonstrated understanding of and commitment to Humber's comprehensive safety programs and practices, including staff patient, and environmental safety, and participation in regular in-service education in this area; and
- Ability to be flexible and effectively work in a busy environment with changing priorities and tight deadlines
- Demonstrated initiative in improving work skills and processes
- Demonstrate competency in a variety of skills including (but not limited to), intramuscular injections, intravenous therapy initiation and discontinuation, wound care dressing, medication administration, etc.
- Demonstrated proficiency in initiating I.V therapy, I.V push meds and venipuncture

Position Responsibilities:
- All aspects of care for patients receiving hemodialysis with a primary nursing focus
- Providing education and follow-up to all hemodialysis patients
- Providing primary care to assigned patient that includes but not limited to ensuring plan of care is developed in collaboration with multidisciplinary team, updated and communicated with team in a timely manner
Qualifications:
- Current RN registration with the College of Nurses of Ontario

- Minimum two years recent Hemodialysis experience is required.

- Design, develop, deliver and maintain clean, secure and efficient reports, visualizations, dashboard solutions using BI and Reporting Tools
- Specify and build dashboards, scorecards, tools, and reports that support strategic and operational goals
