If yes, we invite you to read the details below then apply.   Salary Range:  Location: Humber River Hospital – all sites Hours of Work: Days Employee Group: Non-union     Job Summary: The Clinical Analyst, Health Analytics works closely with Directors and other stakeholders to seek out requests, understand and document the underlying business need, and leverage the correct analytic / business tools, to empower leaders with the right information. When relevant, the Clinical Analyst will work with other members of the Health Analytics team to develop and document requirements for automated technical solutions, and ensure data integrity, validity and reliability.     Responsibilities: - Meets with clinicians and leaders as required, provide an overview on analytic capabilities to support with user request solicitation. - Attend program, review, utilization and/or budget meetings to identify opportunities to empower program areas with analytics, and identifies deliverables. - Provide dedicated support to identified program areas obtaining an intimate understanding of their operations, supporting identification of operational needs and opportunities where health analytics can support. - Support in the development of key performance indicators (KPIs), scorecards, and/or outcome metrics. - Identify opportunities to automate data, including, but not limited to, data collection, tracking, trending and reporting. - Perform review/evaluation on the best mechanism/solution to support the health analytics at HRH. - Collect business requirements using techniques such as “Voice of the customer”, to distinguish between user requests and the underlying business need. - Translate business needs to operational requirements in analytics (e.g. reports, dashboards, etc.). - Document requirements in use case format and create process models, leveraging principles in human factors, design thinking and usability heuristics. - Identify methodological approaches and the data required to answer specific data science questions; - Collaborate with developers and subject matter experts to identify analytic solutions and analyze trade-offs between usability and performance needs. Act as a bridge between the business community and ICAT for analytics. - Produce high quality ad hoc and standardized reports customized per project, tailored to different end-users (e.g. clinicians, researchers, senior management, and hospital executives). - Pre-process raw data to prepare for analysis. This includes cleaning and merging data from multiple sources, as well as understanding overall data quality. - Assist with writing project protocols including: data extraction, cleaning, predictive model, and validation plans - Assist with writing code to put final analytic solutions into production. - Write documentation and contribute to project repositories so that all work is reproducible. - Validate data collected and reported by the health analytics team. - Validate any output tables, listings or figures generated to ensure accuracy and reliability of analyses. - Completes cleaning/consistency checks and other quality control measures necessary for the final analysis. - Critically evaluate and align the right data and output to business needs, decompose high-level information into details, abstract up from low-level information to a general understanding. - Write HTML, PDF, Microsoft Word, and/or Microsoft PowerPoint reports summarizing the analysis. - Prepare presentations for program areas, and leadership, and/or present finding of analysis. - Perform and document analytic requirements for analytic solutions to be built by the data engineer/developer, including interactive dashboards in BCA, reports with interactive visualizations using libraries like Highcharts, plotly, and other analytic solutions. - Conduct an exploratory and statistical analysis of data to assess feasibility of new data science projects using the R, Python or other programming languages. - Coordinate and write business cases as requested, understanding the business need, collaborating with finance/decision support for financial or staffing data, contribute with additional data, and provide for review to supervisor and program leadership. Requirements: - Undergraduate degree in health care, statistics/biostatistics, health informatics or related discipline required - Master’s degree in health care, statistics/biostatistics, health informatics or related discipline preferred - 5 Years experience in healthcare required - 2-3 Years experience in analytics / using analytic tools required (e.g. SAS, R Studio, Tidyverse, etc.) - Expert knowledge of MS Office Suite (Outlook, Excel, Word, PowerPoint, etc.) - Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with leaders, physicians, inter-professional team members, and other stakeholders - Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology - Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands - Excellent organizational, critical appraisal and analytical skills. - Ability to work independently and as part of a team. - High level of initiative and self-direction. - Excellent attendance and discipline free record is a requirement - Professional Certifications in CHIM or PMP preferred - Regulated Health Professional preferred - Experience with Meditech preferred - Experience with Business Intelligence (BI) Solutions, predictive analytics, machine learning preferred - Experience writing proposals, written submissions, and/or business cases preferred   Why Humber? Provide support to the Manager of MDRD with day-to-day co-ordination of activities, tasks, departmental updates, staff meetings and workload in the reprocessing department across all sites - Assist Manager of MDRD in supervising reprocessing in compliance with safety and materials handling requirements (OH&S, WHIMIS and SDS); supervises safety issues and staff injuries in collaboration with HR and OH&S departments. - monitoring and analyzing instrument inventories to ensure timely, accurate reprocessing, in consideration of delivery outpatient clinic carts, reordering of supplies for reprocessing, - analyzing product data on the supply carts to determine supply usage, turnover rates, and appropriate product mix for each supply cart and vendor - Liaise/Coordinate – - with clinical and other departments to meet supply usage and quotas - with the clinical teams for immediate responses to product recalls, communicate and document recall processes - Monitor, correct and optimize inventory tray levels in clinical areas - Reconcile invoice impacts for all orders - Perform other duties as required. - Assist the Manager of MDRD – - with the development and updating of job descriptions and policies and procedures - As part of our dynamic team, you can lead the way, as we continue our journey towards high reliability care!   Right now we’re looking for a Registered Nurse to work in our Adult Inpatient Psychiatric Unit within the Mental Health and Addictions Program.    Hours of work: Must be able to work all day/evening/nights shift, subject to change based on unit needs Hourly Rate Range: $35.52-$50.85 Employee Group:   ONA Location - Wilson Site   Responsibilities: - R.N. responsibilities for the assessment and treatment of mental health patients in the acute phase of the illness. - Effective use of the primary care nursing model. - Ability to perform mental status examination and crisis intervention. - Resource/team leader responsibilities. - Individual/group counselling, individual supportive intervention and therapeutic group activities - Nurses may be reassigned to other mental health units as per the Collective Agreement Requirements: - Current registration with the College of Nurses of Ontario. - Current BCLS/CPR certification required. - BScN or a degree in health sciences preferred. - Minimum 2 years recent mental health nursing experience is preferred - Formal CPI certification preferred. - Certificate in Psychiatric Mental Health Nursing required or willingness to obtain when eligible to do so.    - Funding/payment for certification is the individual's responsibility with guidance from the Hospital. - Excellent psychiatric assessment and risk assessment skills. - Phlebotomy skills an asset. - Experience with chronically mentally ill patients. - Understanding of functional psychiatric illnesses, addictions and treatments. - Working knowledge of the Mental Health Act and RAI-MH assessment. - Crisis intervention skills. - Excellent interpersonal and organizational skills, with an ability to set priorities and meet deadlines. - Must be able to effectively work in a stressful fast paced acute patient care environment. - Excellent attendance and discipline free record required. They work in collaboration with SLP’s, wound and skin, food services, and other members of the interdisciplinary team to address and enhance the nutritional needs of the patient. Coverage may also include working in various units and programs including inpatient Medicine, Surgery, Critical Care, and the Reactivation Care Centres (Finch and Church locations) as needed. - Providing appropriate assessment and treatment of referred patients/clients within an acute inpatient setting. - Prioritizing patient caseload to meet the needs of referred patients. - Recording client assessment, progress and discharge status. - Recording timely workload measurement data. - Works as a member of the health care team. - Flexibility in caseload and program assignment. - Able to manage patient caseload in a busy acute care setting. - Clinical supervision of clinical nutrition students/interns. - Some weekend and statutory holiday coverage may be required as needed. - Cross-coverage at both the Wilson and Reactivation Care Centres (Church & Finch locations) may be required as needed.   Qualifications: - Bachelors or Masters Degree in Food and Nutrition. - Current registration in good standing with the College of Dietitians of Ontario. - Completion of an internship program and a member (or eligibility to become a member) of the Dietitians of Canada. - 1-2 years clinical experience is preferred. - Experience in TPN an asset - Excellent knowledge of clinical nutrition principles and evidence based nutrition practice - Demonstrated excellent written and verbal communication skills, including an ability to encourage, support and motivate patients and/or their families from a multi-cultural community. - Excellent time management and organizational skills. - Able to work effectively in an interdisciplinary team. - Ability to travel for cross-site coverage may be required. Reporting Relationship: Manager, Integrated Transitions in Care Hours of work: 8hr day shifts including rotating weekends.. Location: Wilson Site EmployeeGroup: NOWU Hourly rate:  $31.087 - $31.566 Responsibilities:   • Works collaboratively with the interdisciplinary team to support the ongoing functioning of the outpatient Assess and Restore program, related to referrals, consultations and follow-ups • Functions to ensure referrals are triaged and appointments are set-up in the appropriate time frame • Responds to patient inquiries, questions or concerns that arise during appointment or via telephone • Performs medication history (BPMH), nursing assessments, monitoring and documentation • Provides health education for patients and families regarding chronic disease management, diagnostic tests and prescribed treatments • Integrates best practice guidelines and evidence based research into clinical practice related to Seniors Care • Maintains abreast of new developments in geriatric syndrome management and Seniors Care with integration of related evidence-based advancements into clinical practice • Liaises with the patient’s primary care practitioner and community agencies as required throughout the care process • Utilizes Resource Matching Referral (RMR) and other referral sources to ensure patients are linked to appropriate resources • Helps guide patients in care planning and navigation of seniors care resources along the continuum of care • Facilitates movement through the trajectory of Seniors Care, ensuring patients are connected with the appropriate services, resources and supports within HRH and in the community • Researches, creates and maintains an accurate and up to date compilation of community resources and supports geared to improving the quality of life of seniors at home • Identifies and prioritizes not only the physical needs, but also the psychological, social and spiritual needs of the patient and/or caregiver • Ensures that the patient and/or caregiver is included in development of the care plan and understands the medical advice or recommendations. • Develops linkages with external partners as appropriate to ensure continuity of care     Requirements:   • Graduate from a recognized Registered Practical Nursing two year diploma program • Current RPN registration with the College of Nurses of Ontario • Current BCLS certification required • Minimum of two years recent experience related directly to Seniors Care required. • Demonstrates knowledge and understanding of community resources for older adults. • Demonstrates knowledge of geriatric syndrome management. • Community experience preferred. • Ability to adapt and effectively work in a fast-paced environment with changing priorities and time sensitive targets • Must possess excellent organizational skills • Exhibit assessment skills and an ability to identify and react quickly to a deteriorating patient.
