Dialysis Liaison
POSITION SUMMARY:
The Dialysis Liaison is an integral member of the Nephrology Program, who applies knowledge, skill and judgment to organize, and provide home haemodialysis treatment in the community within Humber River Hospital’s (HRH) catchment area. The Dialysis Liaison considers individual patient needs, time, supplies, and equipment requirements in consultation with the registered nurse.
RESPONSIBILITIES:
- Assist home hemodialysis patients with treatments within the home setting according to treatment plan while promoting quality of life for patients as directed by the nurse;
- Must complete and maintain related records and documentation;
- Must continuously observe patient and assist with necessary machine alarms; and must report to the primary registered nurse status changes and document;
- Assists team with necessary supplies to provide haemodialysis treatments;
- Responsible for correct use of equipment according to guidelines and protocols; and reports any piece of equipment that is out of order and remove it from use;
- Follows all infection control precautions and protocols under the direction of the Registered Nurse;
- Reports any requests or complaints made by the patient, and families to the Registered Nurse;
- Reports any other concerns or issues to the Registered Nurse as required;
- Works safely and ensure the safety of others by following established safe operating procedures, practices and policies and by reporting hazards and participating in Health & Safety training;
- Maintains confidentiality in all aspects of work;
- Attends and completes all required training, education and in-services as required;
- Participates in Quality Assurance Program, and related quality improvement activities;
QUALIFICATIONS:
- Personal Support Worker certificate or equivalent;
- Shifts are rotating 10hr and 12hrs, Days, Evenings and Weekends
- Minimum 1 year experience in related field;
- Demonstrated comprehensive knowledge of health care practices and techniques;
- Valid driver license in good standing with reliable vehicle to travel within HRH’s community;
- Current BCLS & CPR Certificates;
- Member of OPSWA preferred;
- Effective written and oral communication skills and interpersonal skills to work as part of a multidisciplinary team;
- Must have the knowledge, skill, judgment and ability to work with patients in an understanding, sensitive and compassionate manner;
- Must be a self-directed individual with well-developed organizational skills;

Dialysis Liaison
POSITION SUMMARY:
The Dialysis Liaison is an integral member of the Nephrology Program, who applies knowledge, skill and judgment to organize, and provide home haemodialysis treatment in the community within Humber River Hospital’s (HRH) catchment area. The Dialysis Liaison considers individual patient needs, time, supplies, and equipment requirements in consultation with the registered nurse.

QUALIFICATIONS:
- Personal Support Worker certificate or equivalent;

Reporting Relationship: Manager, Fracture Clinic & Plastics
Hiring Salary Range: $29.984-$38.133

Position Responsibilities:
- Performing interventions focused on the following areas: splint/cast and dressings/bandages application; windowing/wedging casts; trimming, bi-valving casts, bandages and splints; removal/repair/re-enforce casts; removal of sutures/wires/pins/external fixators; application of principles of surgical asepsis; preparing for minor surgical procedures application; setting up surgical field for injections and minor procedures; and adjusting and removing orthopaedic devices.
- Assisting physicians on inpatient units and the plastics clinic with orthopedic procedures, including wound care, aspirations, minor surgical procedures and fracture reductions.
Performing/Participating in a comprehensive assessment of patients’ conditions, and develop an orthopedic care plan in collaboration with the family and the interdisciplinary health care team.
- Assessing/Re-evaluating/Revising the orthopedic plan based on patient condition.
- Processing physician orders for diagnostics, results, etc.
- Documenting care provided in the patient record.
- Providing education related to orthopaedic treatments and discharge instructions/follow-up care to patients and families (e.g. cast care, brace and appliance application, wound care, osteoporosis, injections, crutch use, etc).
- Maintaining inventory of orthopaedic supplies and devices (adjusting quotas, requesting appropriate staff to order- supplies, etc.), ensuring equipment and supplies are adequate, in safe working order, and are reported as needed for repair and replacement. Ensuring supplies are used economically.
- Keeping work area, equipment and supplies hazard free and reporting any health and safety concerns appropriately and in timely manner.
- Ensuring self and others adhere to safety policies and practices related to medical devices, equipment and bio hazardous materials.
- Following hospital guidelines and standards for documentation including incident reporting
- Performing work assignments under the direction of an orthopaedic surgeon, working within scope of practice, and following hospital policies.
- Training/Coaching of Orthopaedic Technicians and students.
- Maintaining CSOT registration and competence by attending in-services, workshops, conferences, physicians’ rounds etc.
Qualifications:
- Graduate of a recognized Orthopaedic Technologist Programme sanctioned by the CSOT, apprenticeship programme, and sponsorship by an Orthopaedic Physician
- Current registration by the Canadian Society of Orthopaedic Technologists (CSOT)

- Unit Aide duties in Emergency Department.

Area of Focus: 13 East Acute Medicine and 14 East Inpatient Stroke & Rehab
Responsibilities:
- Advocate for use of best practices for clinical staff in the Acute patient care environments.
- Guide clinical staff in acute and outpatient areas with the interpretation and implementation of professional and clinical practice standards.
- Ongoing translation of evidence-based knowledge, pertaining to Acute and Rehab Stroke, and Acute Medicine and subsequent application to clinical practice.
- Monitor and evaluate staff and program compliance with professional best practices and standards to identify and resolve issues with the delivery of quality, safe patient care and compliance with professional standards; refer complex issues to Director for resolution; facilitate required practice changes, enabling staff to achieve or exceed
- Develop reports of staff disciplinary actions for submission to regulatory health professional colleges and professional associations in collaboration with Manager and Director.
- Develop project plans, requirements, testing and implementation plans, resources and interdependencies, monitor implementation.
- Collect data on practice needs (e.g. programs, care interventions, policies, standards in clinical area) to identify gaps in nursing practices, competencies, and service quality; develop educational strategies, plans, and content (e.g. policies, processes, technologies) in consultation with unit leadership team and clinical staff; and evaluate post-training outcomes.
- Conduct impact analysis to determine training and education activity and program interdependencies (e.g. technical Meditech systems use, orientation, accreditation, hand hygiene, new or updated care and treatment standards) and keep staff updated with new practices, skills, and certifications in assigned clinical areas.
- Coach/mentor clinical staff; train new staff during orientation and monitor against best practice standards; recommend disciplinary actions for staff noncompliance with professional and operational requirements and standards to Director and monitor outcomes.

Requirements:

Reporting Relationship: Manager, Clinical Programs
Hours of Work: Days, Monday to Friday
Responsibilities:
- Prepare and submit monthly Activity Level Reports (ALR) in collaboration with IS&T/Health Records
- Develop quality flow and reporting processes (e.g. QA process map) in consultation with Manager
- Analyze and coordinate changes to meet newly updated reporting requirements\
- Update data elements file and data source maps in database
- Adjust data with Manager approval to respond to issues
- Discuss changes to funder strategic plans with Manager to analyze impact and transfer knowledge of changes
- Creating meaningful utilization reports using various systems (WTIS, iport, Meditech, abstracted and financial data) to package key monitoring information that would be user friendly and available in real time.
- Providing regular statistical and narrative data on Wait Time activities with interpretation to support the various clinical programs.
- Providing regular statistical and narrative data on Decision Support activities with interpretation to support the various clinical programs.
- Educating health care providers in the benefits of appropriate documentation and identifying the impact of documentation gaps as it relates to wait time and financial Information.
- Assisting in the procurement, testing and implementation of valid and reliable tools for clinical resource utilization monitoring and analysis
- Working in collaboration with the Information Systems Specialist to maintain/update/change dictionary entries of Wait Time Information System as required.
Requirements:
- Completion of diploma or degree in health sciences, health administration, epidemiology, information management, or related programs. A combination of experience and education will be considered.
- Minimum three years experience in database systems and activity level reporting.
- Sound knowledge of medical terminology and documentation systems an asset.
- Demonstrated report writing capabilities.
- Demonstrated computer knowledge/expertise in MS Office i.e. Power point, Excel, Access, Word - spreadsheets, graphs and databases with report writing skills.
- Experience in CIHI data and ICD-10 coding preferred.
- Ability to facilitate, deliver and evaluate training programs.
- Excellent interpersonal, written/oral communication skills, organization and problem solving skills.
- Demonstrated ability to work with hospital staff of all levels and specialties in a positive, cooperative manner.

As an ICU Manager, you will play a crucial role in managing and overseeing the operations of our 6 EAST ICU, ensuring the delivery of high-quality patient care and effective functioning of our Level 3 ICU.
Responsibilities:
Reporting to the Program Director of Critical Care and Oncology, the ICU Manager:
- Provides leadership and direction to the ICU staff, including nurses and other healthcare professionals

- Assist in the development and delivery of education for managers, employees and any other identified stakeholders.

We currently have an exciting opportunity for a Dietary Aide to join our Food Services team at the Finch Reactivation Care Centre facility.
Reporting Relationship: Manager, Patient Food Services
Hiring Rate Range: $23.546-24.011
Shift: PT - Must be available for all rotating shifts including alternating weekends.
Employee Group: NOWU
Location: Finch Site (subject to change)
Position Responsibilities:
- Tray passing, diet changes and preparation and portioning of food items as necessary.
- Regular cleaning and sanitation of work areas and department equipment.
- Working in dish room and pot wash room washing dishes and cooking equipment.
- Removing garbage, recycling and slop from the department.
- Safely and properly handling cleaning chemicals used in the department.
- Ensuring all areas of the kitchen are stocked with clean wares as needed.
- Other related duties as assigned.
Requirements:
- Ontario Secondary School diploma or equivalent.
- Good command of the English language (English reading and writing test will be administered).
- Minimum of 3 years hospital food service experience, including safe food handling and HACCP knowledge preferred.
- Food Handlers Certificate Required.
- Computer Knowledge an asset
- Knowledge of therapeutic diets and nutritional supplements.
- Ability to lift, pull and push weights of 25lbs or more frequently throughout shift.
- Must have a positive attitude and a genuine commitment to quality.
- Ability to work well under pressure.
- Demonstrated flexibility and adaptability to changing work routines or priorities.
- Able to work with minimal supervision. Must be self-motivated and demonstrate initiative.
- Ability to work independently or as a member of a team.
- Demonstrated problem solving abilities.
- Demonstrated excellent interpersonal, communication and customer service skills
- Excellent attendance and discipline free record required.
